Compliance with the Rules and Directives

In Consideration of your approval to provide with one (1) mobile phone at my request, I agree to
comply to the following:

a. I hereby solemnly aver and undertake the responsibility of the following 1 Mobile phone
provided by the Yellow Bus:

b. I hereby solemnly affirm that, if any mobile phone above mentioned is misused under any
circumstance, I am liable for punishment according to the rules and regulations. I will be
solely responsible for any prejudice cause from misuse or mishandling of the mobile phones
and SIM card. Yellow bus will not be responsible for any damage or harm caused by misuse
of the mobile phones and SIM.

c. I hereby agree to pay Rs. 6750 per phone (MRP. of Mobile Phone) if the mobile phone is
damaged/ lost by me or any representor of me.

I hereby undertake the responsibility of all the mobile phones and SIM provided to us by Front
Benchers InfoTech Pvt Ltd.
