RE: Slippery Floor Accident

Dear Jollibee,

The purpose of this letter is to demand payment of money for the expenses I incurred for all my medical
bills.

On December 12, 2017, I went to your branch in Jollibee Cor. Taft Avenue and ordered my meal;
as I was going up the stairs, I slipped and fell on my bottom and flipped down the stairs of your
establishment. I slipped and fell because of the wet floor which had no warning, or any sign placed on it.
Many people witnessed the incident. As a result, I sustained injuries which required medical attention.
Luckily, my family members were there to rush me to the nearest hospital. The sum of all my medical bills
amounts to P100,000 or ONE HUNDRED THOUSAND PESOS. Attached herein are copies of my medical
bills.

I am expecting to receive the payment within 20 days. Failure to do so will force me to take necessary
legal actions to enforce your obligations to pay damages.
