    Dear Sir/Madam:

    In line with our Company’s service improvement program, we will be


    implementing our expansion/maintenance works covered by FTTH PROJECT.

    With this in view, we respectfully request for a RIGHT OF WAY and WORK
    PERMIT/CLEARANCE to install, sell, operate and maintain PLDT facilities and among
    others, allowing us to set poles and install/splice aerial fiber optic cables, terminal
    box/NAP and its accessories, within your area of jurisdiction/subdivision in
    consonance with the company’s service improvement program.

    We assure you that we will abide with all your requirements pertaining to the work
    stated above and that proper safety measures shall be exercised during the entire
    period of implementation.
