As per telephonic discussion with you on 17.10.2017. I am enclosing all reports of my patient
Mr Anand Kumar suffering from Multiple Myloma Stage III as he is not in the
condition to travel & meet you. He is undergoing treatment with Tata memorial hospital
Mumbai since 15/09/2016.

History: Treatment started from 15/09/2016 in Tata memorial Mumbai with symptom excess Loss
of weight, constipation, weakness and mild fever up to 990 F throughout day. Then Doctor Started
weekly chemotherapy and it continued till July 2017.And in this period he recovered a lot & started
feeling better. But now Doctor completely stopped chemotherapy. After stopping chemotherapy
approx 2 month he was OK. But now his health again started deteriorating since few days. He feels
very weak & sleepy throughout day & his body temperature rises in evening up to 990 F. He has now
become very weak and not in the condition to travel.

So, therefore I request you to please see the report and start treatment as soon as possible( Sir for
