JOINT-AFFIDAVIT OF TWO DISINTERESTED PERSONS

We, CLEMENCIA C. TUMALIUAN and FAUSTA M. BANATAO, of legal ages,

Filipino citizens and residents of Atulu, Iguig, Cagayan, after first having been duly sown
to in accordance with law jointly depose and say:

1. We personally know JOEL GUTTAN MALLO who is the son of spouses ARTURO
C. MALLO and NICOLASA GUTTAN of Atulu, Iguig, Cagayan;

2. We likewise know that JOEL GUTTAN MALLO was born on March 01, 1972 at
Atulu, Iguig, Cagayan;

3. We also know that the fact of birth of JOEL GUTTAN MALLO was not registered
with the Local Civil Registrar of Iguig, Cagayan due to excusable negligence as
shown by the hereto attached negative certification of birth; and

4. We have jointly executed this affidavit freely, voluntarily and intelligently in order
to attest to the truth of the foregoing statement.
