the teachers name LET rating, and other information recorded in the PRC Office.
_____4. Certified photocopy of LET ratings obtained in Licensure Examination for Teachers (LET)
Professional Board Examination for Teachers (PBET).
_____5. Certified copy of transcript of records with computed GWA (Certification of GWA).
_____6. Copies of service records performance rating and school clearance those with teaching
experience. If unavailable, the applicant must have submitted a justification citing the reason/s
for unavailability.

_____8. Certified copy of Voter’s ID or any proof of residency as deemed acceptable for the School
Screening Committee
