2014 TO DELIBERATE THE FIREARM ACCOUNTABILITY OF PO3 DONALD D
DANAO, PRESENTLY ASSIGNED WITH THE CIVIL SECURITY GROUP.

SECRETARIAT: No, Sir. NUP PANGANIBAN is on official business; all others members
are present hence we have a quorum sir.
CHAIRMAN: Considering that three (3) members of the Board are present, I hereby
declare a quorum and the proceedings will go on as scheduled. Are you
all willing to perform your assigned duty?
CHAIRMAN: Alright then, may I request the Secretariat to swear all the members of this
Board.
MEMBER/SECRETARIAT: PO3 Danao claimed that the said firearm was stolen inside
his house located at BLK. 60 Lot 40 St., Claire Subdivision, Brgy. Loma
De Gato , Marilao Bulacan last April 23, 2016 between 2:00 to 3:00 A.M.
CHAIRMAN: (To PO3 Danao) Kindly introduce your self and please tell us the
circumstances leading to the loss of your firearm.
PO3 Danao: Good afternoon to all the members of Board of Survey. I am PO3 Donald
D Danao, presently assigned with the Civil Security Group (CSG).
On April 23, 2016 I came home from Laguna at about 1:00 in the
morning. Due to tiredness and sleepiness I just placed my bag containing
my issued firearm and other documents at the top portion of my bed, near
the sliding window.
PCI MANALO: What action did you take, upon knowledge that your issued firearm was
lost?
PO3 Danao: On April 23, 2016 sir , I personally appeared before the nearest Police
station, which is the San Jose Del Monte Police Station to report the said
incident. However, I was directed to report the incident to the Marilao
Police Station because it is the latter that has jurisdiction over the place
of the incident.
PCI MANALO: So, when you reported the said incident to Marilao Police Station, did
you ask for assistance of SOCO team?
PO3 Danao: No sir, but there was an Investigation Report. I also reported the said
incident to the Caloocan Police Station since it is just five (5) meters
away from our residence.
CHAIRMAN: So, when you firearm was lost, your bag was also lost?
PO3 Danao: Yes sir. Since my issued firearm was inside my bag.
PO3 Danao: Six (6) sir, and I am the only male occupant.
CHAIRMAN: Are there instances before that your subdivision has been penetrated by
thieves?
PO3 Danao: Yes sir. That is why, I decided to construct our house up to 3rd floor.
CHAIRMAN: Have you coordinated with the Marilao Police Station, for the possible
recovery of your firearm?
PO3 Danao: Yes sir, I already did. I also made a follow-up for possible identification and
arrest of the culprit, however until now there was no possible lead hence, I
will just pay the said firearm.
PCI MANALO: Sir, I move that PO3 Danao, being an active member of the PNP should
replace the same firearm. It has been the constant observation and
recommendation of COA that the firearm should be replaced with the
same kind and make so that the firearm will be used by the PNP
personnel, because if payment will be made, the amount will go directly
to the National Treasury.
CHAIRMAN: Alright then, before the Board makes the recommendation, is there any
further opinion, suggestion or comment?
(At this time, the applicant was excused from the proceedings)
CHAIRMAN: Now, after deliberation and consideration of the documents submitted, the
Board resolves and recommends that PO3 Donald D Danao be absolved
from his accountability provided that he should replace the firearm with the
same kind and make.
He claimed that the said firearm was stolen inside his house located at BLK. 60
Lot 40 St., Claire Subdivision, Brgy. Loma De Gato , Marilao Bulacan last April 23, 2016
between 2:00 to 3:00 A.M.
CERTIFICATE
I CERTIFY that the loss of the article of public property shown above,
and /or on attached sheets, was lost in the manner stated and without fault or
neglect on my part, and that the attached sheets have been examined by me
personally, has never been inventoried or re-issued to other end-user and
is, in my opinion, missing/lost.

FINDINGS. The members of the Board have examined all available evidence and
valued the particular firearm at Php 46,585.00 as per PNP Circular 001-02 or 2013
PNP Standard Cost Valuation for Issued and Loaned Firearms.
“SO RESOLVED”.
BOARD OF OFFICERS
It is further recommended that the subject firearm be declared lost and dropped
from the list of PNP issued firearms and maintained in the list of lost PNP Firearms for
future recovery.
A copy of the Board of Survey Resolution has been forwarded to the Acting
Chief, FEO with a request that the said firearm be included in the database of
wanted/lost PNP firearms to prevent its inclusion on future firearms amnesty or any
means of registration.
