WAIVER

The College of Criminology of Foundation University, Dumaguete City, is issuing this

waiver in connection with its On-The-Job Training (OJT) Program Offered this Second
Semester AY 2017-2018 and which is in line with the current curriculum.

The Student-trainee ________ ______ is officially enrolled in the

Criminology program and his/her parents or judicially appointed guardian acknowledge
that permission is granted to participate in the said program, however, is subject to the
condition, that the university nor the agency where they are assigned will not assume any
responsibility whatsoever for any injury or accident which may happen to the student-
trainee, within or outside the premises of the program area, during the period of said
program. It is understood that there is no employer-employee relationship between the
agency and the student-trainee.

This waiver will be in effect for the duration of the program from November 27 to March
16, 2017.
