SUPPLEMENTAL AFFIDAVIT OF GEOGRAPHICAL PORTION

I, FLORA MAY A. ENRIQUEZ, of legal age, Filipino, single and residing at

Brgy. Liloan Ormoc City, Philippines, after having been duly sworn to in
accordance with law depose and say:

1. That I was born on MAY 13, 1984 at Brgy. Liloan Ormoc City;
2. That my parents are Oscar Enriquez and Estrella Arcelo, both of
Filipino Citizen and a resident of Ormoc City Philippines;
3. That in my Certificate of Live Birth, the Geographical Portion of
Province and Municipality was not completely filled out;
4. That through this Affidavit, I am stating and affirming that I was born
in Brgy. Liloan, under the City of Ormoc, Province of Leyte.
5. That I am executing this Supplemental Affidavit of Geographical
Portion to attest the foregoing facts to be true and correct and for
whatever legal purpose this may serve.
