Dear Sir/Madam,

I would like to apply for Schengen visa to cover my intention to travel to Italy from
December 17 to 30, 2017.

My main purpose of travel is to visit my Italian friend/family. I also look forward to

witness in actual the magnificent Colosseum and Trevi Fountain for Italy is known for,
experience the food and learn the Italian way of life and culture. During my stay, we also
wish to spend a holiday in the Netherlands to visit its attractions.

Please find the following documents to support my visa application enclosed herewith:

1. Application form, duly dated and signed with passport size pictures.
2. Passports showing my travel experience.
3. Travel insurance with benefits coverage of 30,000 Euros.
4. Proof of Sponsorship and/or private accommodation, legalized at my partner’s
municipality.
5. Certified true copy of my mother’s Employment Certificate and Payslips as proof of
his income/sponsorship.
6. Certified true copy of my mother’s Passport (with PH stamps).
7. E-ticket reservation for my flight via PAL for Manila – Abu Dhabi-Rome – Abu-Dhabi-
Manila [April 28-May 24, 2014];
8. Employment and leave certificates showing approved leaves and the date I am
expected to return to work.
9. Payslips, Savings Account and ITR as proof of my income.
