Explanations for your queries raised are as under:-

1) REQUIRE CLARIFICATION FOR NON-INTIMATION AND

DELAYED SUBMISSION -

Baby was immediately admitted to hospital after delivery and the intimation of my
delivery was given to you on 08.03.2017 before the delivery.
I have submitted my medical bills on 24.03.2017 well in advance.

2) REQUIRE ORIGINAL REPORTS DONE DURING HOSPITALIZATION (RS. 820/-),

ATTESTED BY BANK

I have submitted original reports alongwith the other reports.

3) REQUIRE ORIGINAL AUTHENTICATED CASH PAYMENT RECEIPT OF FINAL

HOSPITAL BILL, AS WE HAVE RECEIVED FINAL HOSPITAL BILL ONLY

Cash payment receipt and final bill are the same. On the bottom line of final bill it
is mentioned that payment is made in cash and it has been affixed by revenue
stamp and signed by the concerned authority
