1. GENERAL

1.1 Definitions & Abbreviations

1.1.1 Definitions

The words “will”, “may”, “should”, “shall” and “must” have specific meaning as follows:
“Will” is used normally in connection with an action by the Company rather than by the Contractor.
“May” is used where alternatives are equally acceptable.
“Should” is used where a provision is preferred.
“Shall” is used where a provision is mandatory.
“Must” is used only where a provision is a statutory requirement.

1.2 General Scope Work, Objectives and Special Requirements

1.2.1 General Scope of Work

Grissik-Pertagas Ultrasonic Custody Metering and Pipeline Tie-in Project scope including Engineering,
Procurement, Fabrication, Construction/Installation, Pre-commissioning, Commissioning/Start up
support, Integration, Certification support, and Performance test support of tie-in header, new ultrasonic
metering package, and pipeline tie-in facilities to PT Pertagas pipeline system.

The general scope including, but not limited:


1. Fabrication, installation, replacement and extension of existing 30” ANSI/ASME 600# South
Sumatra West Java (“SSWJ”) metering header during the shutdown window;
2. Installation of 20” double isolation and bleed valve in the SSWJ extension header to allow tie-in to
the Grissik-Pertagas ultrasonic metering package during the shutdown window;
3. Fabrication, installation, and update SSWJ balance and flare header including isolation valves
during the shutdown window;
4. Lifting and installation of ultrasonic metering package to the foundations and installation of
interconnection of piping, electrical, instrumentation, etc. The ultrasonic metering package is in the
form of a complete skid consist of, but not limited to 20” ANSI/ASME 600# inlet headers
completed with the sampling probes, five (5) unit 10” class 600# manual valves completed with
limit switch, five (5) unit 10” class 600# on/off actuated valves, 2 x 100% ultrasonic custody
metering, upstream/downstream spool and flow profiler, construction spool, analyzer tapping
complete with fittings, structural skid and certified lifting gears, instrumentation junction boxes,
electrical junction boxes, skid cable trays, skid cables, grating floor, instrumentations (pressure
transmitter/pressure indicator/temperature transmitter/temperature indicator), balance line valves,
de-pressuring valves, outlet header, and all accessories. In addition to the above Contractor shall
install the ultrasonic metering package loose item which consist of metering panels, flow
computers, diagnostic system, Human Machine Interface (“HMI”) metering station, switches,
firewall, PDC server, maintenance laptop completed with software relevant and other accessories

for metering system integration with the existing H2O analyzer, existing H2S analyser, existing Gas
Chromatography (“GC”) and all other accessories.
5. Installation of pipeline tie-in facilities to the Pertagas pipeline system. The facilities shall include
but not limited to 16” pressure control valves, 20” manual valves, 20” check valves, 20” ESDV
valves, 20” pipes, fitting and flanges. The facilities shall be connected to the Pertagas pipeline
battery limit.
6. Supply and installation of civil/structural which comprise of ultrasonic metering package
foundations, pipe support foundations, ultrasonic metering shelter, pipe support structurals,
instrumentation support structures, electrical and instrumentation junction boxes supports, cable
trays support, control room modification, provide new drainage, repair existing drainage, concrete
flooring, gravelling and re-instatement.
7. Demolish the existing fences and supply and install the new fences up to 200 meters.
8. Supply and installation of electrical Work including cabling, power to the ultrasonic metering
package, tie-in to the existing UPS system, cable tray, power panel, power junction boxes, lighting
system, breakers, etc.
9. supply of materials and execute the instrumentation installation Works including all control room
integration, provide HMI metering and integration, provide tie-in to the existing instrument air
system, automation work of DCS, SIS, site instrumentation, and integration to the existing SSWJ
metering and new metering control system;
10. provision of heavy equipment during the construction period which shall include but not limited to
construction crane, TMC, transportation car, etc.
11. supply of materials and execute the installation of process safety equipment which shall be
completed with wheel firefighting equipment, safety sign, eye wash installation, etc.
12. Develop detail engineering for process, process safety, piping, civil/structural, instrumentation,
metering, and electrical as required based on the Company distributed FEL-3 engineering design
under Exhibit G of this Contract;
13. Supply of bulk materials and execute the piping fabrication, post weld heat treatment (“PWHT) as
necessary,;
14. modification of the existing structural, pipe support, and associated modification work to
incorporate the new additional facilities;
15. provision of construction & fabrication bulk materials and consumables and
construction/installation equipment, tools, manpower;
16. provision of materials and execute the site installation of all electrical system including grounding
system, lighting fixtures, power system, cabling, termination, junction boxes, etc.

17. provision of materials and execute the site installation of all instrumentation devices including but
not limited to transmitters, gauges, automation system (PCS/SIS), instrument cables, cable trays,
instrument tubing, junction boxes and termination;
18. provision of materials and execute the civil and structural work including metering shelter,
platforms, ladders, foundations, pipe supports, instrumentation stations, electrical structural support;
19. Execution of pre-commissioning work for the whole system;
20. Execute the programming and/or software installation, update and programming required to
accommodate/communicate new ultrasonic metering package configuration with existing facilities
and also instrumentation interconnection required to accommodate new ultrasonic metering package
configuration compatible with the existing analyzer system. Perform any work required to
interconnect new ultrasonic metering package, flow computer and DCS/SIS, and manage data
required by Company’s Customer properly/satisfactory;
21. Execute the commissioning/start up, performance test, running series, acquisition data, and
certification support and assistance;
22. Support Company’s other contractor to obtain the certification for ultrasonic metering package
issued by DIMET/DITMET and MIGAS, SKK Migas and Company’s customer. Contractor shall
be responsible for obtaining Surat Keterangan Pengujian (“SKP) issued by DIMET/DITMET and
Surat Izin Penggunaan (“SIP”) issued by MIGAS. Execute the performance test of overall system
including ultrasonic metering package system.
23. Provision of temporary facilities, accommodation and transportation for all Contractor personnel in
accordance with Company standard no. ID-G-BU-GG0-STR-GN-00-0013 under Exhibit G
24. Provision Company site office for up to ten (10) personnel in accordance Exhibit F of this Contract;
25. supply and procure all materials which are not listed in Exhibit J – Company Furnished Material;
26. provision of meeting venue and procedure of Gas Delivery with the capacity of thirty (30)
attendees. The meeting will involve the following parties : Contractor, Ultrasonic Metering
Packager, Government Authority, Company, and Company’s Customer. The meeting will be
conducted about three (3) times at minimum with the duration of two (2) of each meeting batch.
The meeting venue shall be in five (5) stars hotel located in Jakarta, Bandung, Bogor, Palembang
and other place in Indonesia as approved by Company.
27. Conduct the shutdown and commissioning readiness workshop located in Palembang. Contractor
shall provide the meeting venue, meeting kit, accommodation, stationary, materials, etc. The
meeting venue shall be in five (5) stars hotel.
28. Provision of nitrogen for shutdown activities, pre-commissioning, commissioning/start up, and
performance test.

29. Contractor shall comply with the basis of design document No: ID-N-CG-PP0-BOD-PL-25-0003
“Basis of Design” which is distributed under Exhibit G.
30. Provision of all permits, licences and the like, required to perform the Work;
31. Provision of requirements of any Governmental Authority having jurisdiction over the Work;
32. Endorsement (verification and validation) of Company’s engineering documents, existing as-built
and all Company Supplied Data within ten (10) calendar days after the Effective Date;
33. Completion of the detailed engineering within thirty (30) calendar days.;
34. Perform the Kick of Meeting at Jakarta within seven (7) calendar days after Effective Date, Site
Kick off Meeting, Hazop, construction Hazid, constructability review within thirty (30) calendar
days after Effective Date which the venue shall be located in Palembang;
35. engage with the Company’s and Contractor’s vendors and ensure the availability of vendor
representatives/field service engineers during the execution installation, tie-in, pre-commissioning,
and commissioning/start-up activities;
36. Preservation of Company Furnished Material and Contractor Furnished Material;
37. Provision of all direct and indirect labour, supervisor and management level with the proper
compensation which include the wages and salaries and associated payroll costs (overtime
premiums, insurance contributions, health plans, pensions, employers liability or other insurances,
sickness benefit, holidays, bonus) and all costs of labour changeovers;
38. mobilization and demobilization of all Contractor’s personnel, tools, equipment, and other supplies
necessary to complete the work;
39. Interface management (both internal and external);
40. Provision of adequate site housekeeping and material disposal during and upon completion of the
Work to avoid environmental damage in accordance with the local and national rules and
regulations;

1.2.2 Contract Objectives

The objective of this Contract is for Contractor to perform the Work, in accordance with the Contract
Schedule. The Work shall be executed in a manner that meets the following Key Performance Indicator
(“KPI”):
1. Cost : meet or below Contract Value;
2. Schedule: meet or below Contract Schedule milestone and progress;
3. HSE:
a Zero Total Recordable Rate (“TRR”);

b Zero asset damage; and


c Zero environmental damage.
4. Quality: Zero non-conformance;
5. Production: Zero existing facilities Lost Production Opportunity (“LPO”);
6. Ethics: Zero violation; and
7. Auditable.

1.2.3 Special Requirements

1. Any or all exceptions or deviations from Company’s approved technical requirements and
specifications must have Company’s prior written approval in accordance with Exhibit I - Change
Order;
2. In case of conflict with Exhibit G - Company Supplied Data as a result of complying with this
Exhibit or in case of conflict within this Exhibit, the most stringent requirement shall apply;
3. In case of conflict with Codes and Standards referenced in Technical Documents as a result of
complying with this Section, the conflict shall be brought to Company’s attention for resolution and
approval in writing. In all cases the most stringent requirement shall apply;
4. Contractor shall satisfy itself on the actual site condition by conducting all necessary action to
obtain all data and information required to perform the Work. Company shall have no responsibility
on the actual site condition;
5. In all cases, Contractor shall review, assess, verify, revalidate and satisfy itself on all data and
information provided by Company. Company shall have no responsibilities on the difference
between the provided information and actual condition;
6. The presence or absence of Company comments or approvals in all cases and/or any aspects of the
Work shall in no way relieve Contractor of its obligations, liabilities, or responsibilities under the
Contract;
7. Contractor shall correct defects in its work at its own cost;
8. Upon state otherwise, all documents/deliverables prepared by Contractors shall be in English
language;
9. Any impact due to delay in obtaining approvals, permits, licenses, permissions, certificates, and the
like from any parties, shall be at Contractor’s own cost and responsibility; and
10. In the event Contractor fails to comply with Company’s requirement as stipulated in this Contract,
Company shall have the right to immediate stop all or any part of the Work. This stoppage of Work

shall not be a basis for a claim by Contractor for extension of the Work Schedule, additional cost
compensation, or other damages.

1.3 Structure of Scope of Work

Company has defined the main workpacks (elements) of the Contract objects as shown in the tables
below:

1.4 Work Site

Contractor responsibilities at the Work site shall include, but not limited to, the following:
1. Some of the Company facilities have live operations, therefore all Works within the facilities will
be controlled by the Company Operations Department and shall be executed by Contractor with
high standards for safety and planning to minimize upsets to plant operations;
2. Contractor is responsible for becoming familiar with and adhering to rules and procedures provided
in the Bundling Simultaneously Operations Grissik, Gelam, Sumpal, Dayung (SIMOPS - GGSD);
3. All Works inside Company facilities shall be performed according to Company HSE requirements
as referred to Exhibit M of this Contract;
4. All Works inside Company facilities shall be in accordance with Company Permit-To-Work (PTW)
system. Contractor shall have dedicated PTW coordinator on site at all times during the execution
of the Work;
5. Contractor Group Personnel working at the work site will be required to adhere to Company HSE
requirements for rotation of field personnel. Maximum rotation schedule will be twenty-eight (28)
days on followed by fourteen (14) days off. In no case shall any person be on site more than twenty-
eight (28) days, without Company’s prior approval;

6. Any heavy and lifting equipment and related special measurement tools including its operators to be
used within the Work Site shall refer to MIGAS’s certification, as applicable. Contractor to provide
copy of the certificate to Company. Company will use the Company’s standard evaluation during
hiring the heavy lifting equipment including Company third party inspection, Company technical
authority, and the training that required for operator.
7. Contractor Group Personnel shall comply with the minimum qualifications for crafts as dictated by
MIGAS or authorized government bodies and Company HSE requirements as referred to Exhibit M
of this Contract;
8. Contractor shall make themselves familiar and aware with the existing condition of Company’s
facilities located in the Work Sites. Company shall have no responsibilities whatsoever on the
difference between the Company’s provided information and the actual condition;
9. If requested by Company, Contractor shall do the best effort to execute all the work during the
Corridor Block Shutdown for inlet header and valve installation. Contractor shall provide all
preparation work, survey, provide evaluation, and provide all manpower, tools, equipment, services,
and effort to success shutdown tie-in work safely and on schedule; and
10. Failure to adhere to Company’s rules and procedures and requirements specified in this Contract
and its Exhibits will result in shutdown of contract activities until full compliance is confirmed and
accepted by Company. Any cost occurrence as a result of Contractor’s incompliance shall not be
charged to Company and no change in the scheduled completion date will be approved by
Company.

1.5 Project Execution Plan

1. Contractor shall implement the Company’s approved Contractor’s Project Execution Plan;
2. Contractor shall prepare a Project Execution Plan (PEP) for the Work and submit for Company
approval within ten (10) Business Days from the effective date. This shall include the progressive
development, for Company’s acceptance; of the various lower-level plans and other documents that
are described in the execution plan and which address specific topics in greater detail. Such plans
and other documents shall cumulatively set out all Contractor’s management, planning and
scheduling, internal control, quality, safety and other systems and processes required to ensure the
proper performance and completion of the Work in accordance with the Contract;
3. Contractor shall be responsible for management interface between engineering, procurement and
construction components and between disciplines within each components including with

Contractor partners, Subcontractors and Vendors. Additional work as a result of failure of interface
management is not ground for extension of Contract Schedule and/or increase in Contract Value;
4. In developing the PEP (Project Execution Plan), Contractor shall refer and follow the requirements
as stipulated in Exhibit F and shall demonstrate that it has a well-structured and adequately staffed
organization for managing all aspects of the Work. Contractor shall present its proposed
organization chart for the Work taking full consideration of the overall objectives of the project. In
addition to the summary chart, separate charts may be presented for each section and work phase.
The organization charts shall show all the key positions;
5. Contractor shall update the execution plan as necessary during the performance of the Work,
securing Company’s acceptance of any proposed deviations prior to their implementation. A
deviation will only be applicable where proposed actions fall outside the boundaries described in
the execution plan. Contractor deciding, with Company’s agreement, between alternative courses of
action that are described in the execution plan along with full details of the basis on which the
decision would be made in accordance with the Contract Schedule, will not be considered as a
deviation requiring updating of the execution plan; and
6. Contractor shall carry out the entire Work in accordance with this Contractor’s PEP. Failure to
comply with the agreed PEP shall be a cause for Company to withheld related milestone payments
at Company's sole discretion.

1.6 Contract Schedule

Contractor shall have carefully planned all activity related to the execution of the Work as specified in
this Contract. Failure to identify any activity that leads to late completion of the Work as per Contract
Schedule in Exhibit H of the Contract subject to Delay Liquidated Damage (“DLD”).

Contractor shall perform and complete the Work in accordance with the agreed Contract Schedule. The
Contract Schedule shall be prepared in Primavera software version 6.1 in accordance with Exhibit F of
the Contract. The Contract Schedule shall be submitted for Company’s approval within fourteen (14)
calendar days from the Effective Date.

1.7 Conformance of the Work

All engineering, procurement, fabrication, construction, installation, tie-in, pre-commissioning, and


commissioning/start-up shall conform to the highest standards expected of international contractors
engaged in the oil and gas industry, and the specific requirements of this Contract.
All materials and equipment/units which will become Company’s assets shall be supplied by Contractor
in new condition in accordance with this scope as well as Company approved drawings, data sheets,
specifications, reports, and procedures, and comply with Company’s Quality requirements.

The Work and all components thereof shall be engineered and designed for an onshore tropical
environment including selection of paint materials. No deviations from the Company Supplied Data
and/or Contractor issued engineering data approved by Company are to be made by Contractor without
Company’s prior written approval. Nonetheless, Company’s approval does not release Contractor from
obligation to fulfill requirement as stipulated in the Contract.

2. WORK PACKS

2.1 Project Management, Administration and Services

Contractor shall, in conjunction with Company, manage, perform and administer the Work in
conformance with the Contract. Contractor shall diligently track the planned and actual status of all
Work at all times. Contractor shall promptly take all actions necessary to correct or avoid deviations
from the Contract.

Contractor shall have an efficient management organization, fully supported by effective systems and
procedures, to ensure that all Work are performed to meet fully the quality, schedule, safety,
environmental, budget and other requirements of the Contract.

Contractor is responsible for the proper management and execution of the Work, wherever performed,
and it shall have appropriate representation at all Sites to ensure the safe and timely performance of all
elements of the Work in accordance with the quality standards and other requirements set out in the
Contract.

2.1.1 Project Management and Administration

2.1.1.1 Project Control

2.1.1.1.1 General

Contractor shall ensure the satisfactory control and administration of all aspects of the Work by
providing personnel, facilities, and resources in accordance with the Contract. As a minimum, the
following points shall be properly addressed and action:
1 analyse, evaluate, coordinate, and expedite related job function;
2 analyse and optimize resources;
3 identify project critical path and report it for Company’s attention;
4 record project meetings and circulate the minutes of meeting;
5 fully conversant and comply with QA/QC and HSE for incorporating Contract requirements;
6 establish consistent method for risk management (risk identification, risk assessment, risk
mitigation including contingency development);
7 fully conversant and comply with contract terms & conditions and administrate contract; and
8 prepare and circulate project status, schedule, and performance report.

2.1.1.1.2 Project Control Documents

Contractor shall prepare and submit for Company’s approval the following project control documents
within fourteen (14) calendar days from the Effective Date:
1 Contract Schedule;
2 Project Control Procedure;
3 Progress Measurement Procedure; and
4 Change Management Plan.

2.1.1.1.3 Planning and Scheduling

Contractor shall utilize Primavera version 6.1 for its planning, scheduling, and resourcing activities.
Contractor shall describe in detail the principles, assumptions and methods used to plan and develop the
Project Baseline Schedule.

Contractor shall develop a logic network resource loaded (upon Company request), Contract Schedule
level 4 in sufficient detail to plan all Work activities, identify progress, evaluate progress, assess
resources, and establish priorities. Contract schedule shall reflect all downtime, which will include but
not be limited to vacation period, mechanical down time, and weather downtime (but excluding
compensation in respect of weather down time). Contract schedule shall also reflect seven (7) calendar
days review cycle by Company of Contractor deliverables.
Contractor shall identify potential risks to the schedule critical elements and shall develop contingency
plans to ensure prompt implementation of pre-emptive and corrective actions whenever they are needed.
Contractor shall not re-baseline the agreed Contract Schedule unless formally agreed by Company in
writing.

2.1.1.1.4 Progress Measurement and Reporting

Contractor’s progress measurement system shall be based on physical completion of the entire Scope of
Work. The overall progress calculation is the weighted sum of deliverables individual progress
measured according to the agreed upon rules of credit.

Contractor shall follow Exhibit F for the detail progress measurement calculation and reporting
requirement. All Progress Reports shall be submitted in the format and content to be agreed by
Company and Contractor in writing. A native format electronic file shall be submitted with any report
provided to Company.

2.1.1.1.5 Change Management

Contractor shall establish a change management process as part of the cost control system. The process
shall specify how changes are registered, approved and implemented. For each proposed change,
Contractor shall identify all impacts of such change, including impacts to schedule, cost, and other
contract terms for Company review and approval. A comprehensive description of Contractor’s change
management process shall be included in the Change Management Plan.

Contractor shall include in the change management process steps to ensure the early identification and
prompt reporting to Company of potential changes. Contractor shall identify changes, categorize them,
and address all cost impacts as one of the following: Contractor shall maintain, update and issue to
Company on a monthly basis a Change Register which includes a listing of Deviation Notices, Change

Order Request, Change Order, and Change Order Finalization being developed. The Change Register
shall provide a status for each entry (approved, pending, rejected or disputed). Contractor shall follow
Exhibit I of this contract for Change management workflow.

2.1.1.1.6 Document Control and Information Management

Contractor shall operate a computerized document control system to monitor the status and control the
flow of all documents. The system shall be the repository for all Contract related documentations and
correspondences and the like, including those from Suppliers and Subcontractors. All documentations
therein shall become the property of Company and shall be handed over to Company upon completion
of the Work. Company shall have the right to remove all or part of the records held therein.

Contractor may be required to adhere to Company’s online document repository system, in lieu of or as
well as setting up and operating Contractor’s own computerized document control system. Specific
requirements for engineering document control shall refer to approved Engineering Management Plan.

Adequate clerical assistance shall be assigned by Contractor to ensure that all incoming documentations
and correspondences are available to the project personnel on a timely basis and that all records are
current. The system shall be structured to allow Company immediate access to information. The
document control system shall be structured to allow tracking of queries and responses. A document
withdrawal system shall be established to ensure that information is not misplaced.

Contractor shall retain copies of all documents for a period of three years following Final Acceptance,
and shall make such records available to Company upon request. Contractor shall submit for Company
approval a Document Control Procedure. At a minimum this shall include list of all project deliverables
(e.g. reports, specifications, drawings, procedures, supplier data, etc.) and procedures for:
1 Document and revision numbering referring to Exhibit G - Company Supplied Data;
2 Identify and track all documents issued by Contractor, Subcontractors and Suppliers in the form of a
Document Register;
3 Company’s review requirements for documents prepared by Contractor, Subcontractor and Supplier
(identifying documents to be reviewed, number of reviews, time durations and list of reviewers);
4 Identify and track all documents issued to Company, highlighting any late responses from Company;
5 Review all Subcontractor and Supplier documents before submitting to Company;
6 Ensure that the latest revision of the drawings and specifications is utilized in execution of the Work;

7 Ensure the final documentation is received from Subcontractors and Supplier prior to making final
payments;
8 Verify the completion and issue to Company of as-built drawings, specifications and other as-built
documentation; and
9 Ensure the compliance with requirements of Company Document for Operations.

Contractor shall provide Company with a full electronic version of all data developed by Contractor
Group, preferably stored on CDs or other media suitable for long-term storage and ready retrieval of
data and acceptable to Company. Such data shall include all databases, reports, specifications, CAD
drawings, plans and schedules, manpower and man-hour data and any other documents required under
the Contract. Such data shall be in common format native files that can be easily accessed and further
used during other detailed engineering or other works. Read only data and .pdf file are not acceptable.
Contractor shall follow Exhibit F for further detail document control and Information Management.

2.1.1.2 HSE Management

2.1.1.2.1 General

Health, safety, and environmental protection are core values of Company. Without exception,
Contractor shall pay the highest attention to health, safety, and environmental protection. In performing
the Work, Contractor shall comply with the Company HSE requirements as set forth in this exhibit and
Exhibit M - HSE Requirements. In relation to HSE, in case of conflicts, the requirements of Exhibit M -
HSE Requirements shall govern.

2.1.1.2.2 Project HSE Plan

Contractor shall prepare and submit for Company’s evaluation a Project HSE Plan during bid period.
Project HSE Plan shall be updated from the Effective Date and shall be re-submitted for Company’s
approval within fourteen (14) calendar days from the Effective Date. Contractor shall prepare a Project
HSE Plan in accordance with Company HSE Plan Guideline – No.ID-G-BU-00000-00000-00457-
Preparation and Approval of Project HSE Plan Standard.

In addition to Project HSE Plan, Company will prepare a SIMOPS Plan for all site works including
construction, installation, tie-in, pre-commissioning, and commissioning/start-up. A SIMOPS workshop

involving both Company’s and Contractor’s personnel will also be conducted to develop this SIMOPS
Plan. The SIMOPS Plan will include the minimum:
1 a list of applicable legislation related to occupational health and safety;
2 a description of the Work to be performed, including other non-project related operations and
maintenance activities at Grissik Plant which may run simultaneously with the Project;
3 area classification for each type of construction work;
4 a summary of the organisational arrangements as well as roles and responsibilities;
5 a description of emergency response arrangements;
6 procedures for any heavy lifting to be performed; and
7 a summary of risk assessments performed and the means in place, both hardware and management,
to control each identified risk (particularly for performing hot work inside live production facility).

2.1.1.2.3 Emergency Response Plan

Contractor shall prepare and submit for Company’s evaluation an Emergency Response Plan within
fourteen (14) calendar days from the Effective Date to cover all phases of the Work at each location.
Contractor shall ensure that all of its personnel that have designated roles in the plan receive satisfactory
training in such roles, and that exercises are conducted periodically to test the effectiveness of the plan.
The Plan shall be able to bridge Contractor’s Emergency Response Plan with existing Company
Operation’s Emergency Response Plan.

Contractor shall provide at least standby trained paramedics in the Work site throughout the project
duration and all other requirements stated in Exhibit M – HSE Requirement. Contractor may utilize
Company site clinic and doctor on duty with agreement from Company Representatives. In the event
that any Contractor personnel at site require emergency medical evacuation, Contractor shall provide
emergency transportation from the site to the nearest place that can provide suitable medical treatment.
The arrangement shall be communicated in the Contractor’s Emergency Response Plan.

2.1.1.2.4 Environmental Management

Contractor shall pay the highest attention to the environment protection in carrying out the Work.
Contractor shall carry out proper environmental management to ensure that the Work is executed so as
to protect the environment in compliance with all requirements of the Contract including the project
specific Environmental Management Plan. Environmental Management Plan (particularly Waste

Management Plan) shall be prepared and submit for Company’s approval within fourteen (14) business
days from the Effective Date.

2.1.1.2.5 Permit to Work

During Construction period, Contractor shall perform daily safety briefing, safety tool box meeting, and
obtain daily permission from Company, through Company approved Permit to Work (“PTW”) System.
Contractor shall have dedicated PTW Coordinator on Work Site at all times starting from initial
mobilization of Contractor’s resources up to completion of the Work or otherwise advise by Company.

2.1.1.2.6 Personnel Protective Equipment

Contractor shall provide all workers including Subcontractor and Suppliers with complete Personnel
Protective Equipment (“PPE”) in accordance with Company Specification No. ID-G-BU-00000-00000-
00417 - Personal Protective Equipment. Special PPE for mercury contaminated environment shall be
provided by Contractor for some personnel to perform the Work.

2.1.1.2.7 HSE Personnel and Competency

Contractor shall have a designated HSE Advisor/Coordinator and adequate Safety Inspectors to oversee
the Work at all times, fully equipped with suitable PPE complied to Company’s standard and Gas
Detectors for hazardous materials, i.e. CO, Oxygen, H2S, Hydrocarbon, and Mercury. A minimum site
ratio as per Exhibit M - HSE Requirements shall be maintained at all times during the contract duration.
This will be enforced as a contractual requirement.

Contractor’s personnel shall meet Company HSE minimum requirements and to be reviewed,
interviewed and approved by Company prior to be assigned to work in Company Facilities. Contractor
shall also provide HSE trainings for Contractor’s personnel as per Training Matrix in Exhibit M – HSE
Requirements.

2.1.1.2.8 HSE Recognition Program

Contractor shall develop recognition program and implement it to encourage safe working environment
for all workers. The recognitions shall be awarded to an individual, a team or everybody contribute to

the safety. Specific milestone of achievement shall be assigned by Contractor and approved in advance
by Company. The recognition shall be minimum consist of the annual weekly recognition, monthly
recognition, middle project progress, project big milestone include safe shutdown work, complete
ultrasonic metering installation, safe pre-commissioning, and safe commissioning and start up. The
project completion safety recognition shall be there in term of celebration to all of the team if the project
complete safely.

2.1.1.3 Interface Management

2.1.1.3.1 General

Contractor shall manage and coordinate Work related interfaces, including all interfaces with Company,
Company’s other Contractors performing the Work and Stakeholders. Where it is agreed among
Contractor, Company’s other contractors and Company - or Company, at its sole discretion, decides -
that a party other than Contractor shall have the lead role in managing and coordinating any specific
interface involving Contractor, Contractor shall participate co-operatively in all required interface
activities. Contractor shall carry out all required interface activities to ensure the successful execution of
the Work and all interfacing work performed by other parties.

Contractor shall ensure that components at each physical interface are fully compatible in all respects,
including:
1 Engineering;
2 Materials and equipment supplied;
3 Dimensions;
4 Construction; and
5 Project Stakeholders which includes but not limited to:
a Communities; and
b local Governments / Regulation body(ies).

As described in details below, Contractor shall:


1 Produce comprehensive interface registers for the Work, keeping these fully updated at all times;
2 Ensure that there is a clear and unambiguous understanding among all interfacing parties of their
individual roles and responsibilities;

3 Ensure that Company’s other contractors and Company are kept fully informed at all times regarding
Contractor’s requirements, intentions, and progress in relation to interfaces;
4 Carry out thorough inter-discipline checking of and comment on all interface-related documents and
drawings produced by Contractor, Company, or any of Company’s other contractors to ensure design
interfaces have been properly addressed;
5 Co-operate actively with all relevant persons in relation to interfaces;
6 Ensure that Contractor all relevant parties have an express contractual obligation to co-operate with
one another and with Company and Company’s other contractors to ensure that all interfaces are
accomplished successfully;
7 Arrange, as applicable, and participate in all Work related interface meetings, including inter-
Contractor and inter-discipline technical review meetings;
8 Ensure that all schedule-related aspects of interfacing are satisfactorily managed;
9 Provide technical input to and co-ordinate with all relevant persons as necessary to ensure
consistency and compatibility of all Work-related interfaces;
10 Ensure active and continuous Supplier assistance and input to the engineering, construction,
mechanical completion, pre-commissioning, commissioning, and start-up support phases;
11 Interface directly with Company's nominated Certification Contractor, provide all necessary
engineering deliverables to the Certification Contractor for their review, arrange for all necessary
inspections and witnessing by the Certification Contractor and provide all supporting dossiers as
requested by the Certification Contractor, all as required for the issue of SKPP and SKPI certificates
by MIGAS;
12 Keep Company fully informed at all times regarding any matters related to interfaces that could
affect Company, the Contract Schedule, and/ or the Contract Value, or require a decision or prior
approval of Company; and
13 Interface with project Stakeholders, Contractor shall communicate and coordinate with nominated
Company's point of contact and shall following Company’s Communication Protocol & Line and
align with Exhibit O - Company Business Ethic.

2.1.1.3.2 External Interface Responsibilities

Contractor shall liaise with Company’s other contractors and shall prepare a comprehensive external
interface register defining all interfaces between the Work and the work of other parties, data exchange
procedures, contacts, schedule, and procedures for identifying, tracking, and satisfactorily executing all
interface-related activities.

The external interface register shall be submitted to Company within thirty (30) calendar days from the
Effective Date, for Company’s review and approval. Contractor shall incorporate Company’s comments
and re-submit to Company for approval. Upon Company’s approval, Contractor shall issue the external
interface register to all persons to whom Contractor has an interface obligation, or who have an
interface obligation to Contractor. The register shall identify the person (Contractor, Company, or one
of Company’s other Contractors) that has the primary responsibility for the management of individual
interface items. All need dates for information from one person to another, or for the delivery of
physical items by one person to another, shall be agreed co-operatively among Contractor, Company’s
other Contractors, and Company.

Contractor shall co-operate actively, constructively, and effectively with Company and all Company’s
other Contractors performing work. Contractor shall provide all information and other items required by
other Contractors in accordance with the external interface register and schedule.
Contractor shall keep the external interface register fully updated at all times throughout the
performance of the Work, ensuring that all persons with an interest in the register have ready access to
the current version at all times, preferably by on-line electronic means or otherwise by regular issue of
updates.

Contractor shall establish a schedule of periodic interface meetings, in which all persons having a role
in the co-ordination of interfaces shall participate. The objective of the interface meetings is to ensure
complete mutuality of understanding among all interested parties, and to avoid any possibility of
misunderstandings among Contractor Group, Company, and Company’s other contractors. Contractor
shall also supply and use teleconferencing/videoconferencing facilities to the greatest practicable extent
to allow the most effective interaction among and between interfacing parties while avoiding persons
traveling unnecessarily.

Contractor shall keep Company fully informed at all times regarding its proposed schedule of interface
meetings, and shall amend the schedule as necessary to take account of Company’s comments.
Contractor shall give Company timely notice of all unplanned interface meetings. Company shall
attend interface meetings at its discretion, but in any event Contractor shall keep Company fully
informed regarding the results of all meetings. All meeting results shall be recorded, and copies of the
minutes shall be promptly issued to all participants and other interested persons by the Contractor.

2.1.1.3.3 Internal Interface Responsibilities

Contractor shall manage and coordinate all interfaces among Contractor Group. Contractor’s
responsibilities with regard to internal interfaces shall be the same as for external interfaces, except for
matters not applicable to internal interfaces. Such responsibilities include:
1 Establishment and maintenance of the internal interface register and schedule;
2 Issue and distribution of all relevant documents and drawings to all applicable parties, including
Company;
3 Arrange the interface meetings among all applicable parties, with Company having the right to
participate;
4 Issue the regular status reports to all applicable parties, including Company (if requested); and
5 Company at its sole discretion might decide to be involved in the Contractor internal interfaces,
which might impacted to the overall cost and schedule to this contract. Contractor shall participate
co-operatively in all required internal interface activities and providing status reports as required.

2.1.1.4 Operability Assurance

Operability Assurance (“OA”) is the Company structured process for managing and specifying
operability, reliability and maintainability requirements of the Work from Project inception through to
delivery of the Facilities with flawless transition and handover to Operations.

The OA requirements that shall be implemented by Contractor during the Work are included but not
limited to the following:
1 Document for Operations (“DFO”) documents. Contractor shall update the documents based on
latest existing Company documents (i.e.: PFD, P&ID, Cause & Effect Chart) at the time Contractor
producing the As-Built documents. Contractor shall request formally in written to Company for the
latest existing Company documents 1 (one) month prior to produce As-Built Documents. Upon the
above request, Company will ensure that Contractor will receive the latest existing document
required. For detail of DFO documents, Contractor shall refer to Exhibit G on Document for
Operation (Doc. No. ID-G-BU-00000-00000-00012);
2 Completion Management System (CMS) register. Contractor shall implement completion
management system, shall allocate manpower accordingly and shall refer to Exhibit G on
Construction Completion Management System (Doc. No. ID-G-BU-YY0-PRO-GN-00-0019);

3 Engineering Register documents. Contractor shall implement Engineering Register, shall allocate
manpower accordingly and refer to Exhibit G on Engineering Register (Doc. No. ID-G-BU-00000-
00000-00025);
4 Baseline Thickness Measurement. Contractor shall implement Baseline Thickness Measurement
and refer to Exhibit G on Engineering Register (Doc. No. ID-G-BU-00000-00000-00581);
5 Training Plan. Contractor shall develop the training plan for Contractor’s procured equipment and
subject to Company approval. The training plan shall include the training materials provision, the
training venue, meals and associated training package. Company personal expense is excluded from
Contractor’s obligation; and
6 Commissioning & Start-up Procedure. Contractor shall develop the commissioning & startup
procedure and subject to company approval.

Contractor shall establish and maintain dedicated manpower for the following position aligned with the
Company OA Requirements as stipulated in Exhibit K:
1 OA Coordinator (1 personnel) report (at least available at ED + 4);
2 Construction and Completion Management System (CCMS) Coordinator (2 personnel, back-to-
back at site) (at least available during site construction work); and
3 Engineering Register Analyst (1 personnel) (at least available at ED + 4).

2.1.1.5 Quality Management

2.1.1.5.1 General

Contractor shall carry out its quality management to ensure that the Work is executed to meet all the
requirements of the Contract, with the objective of eliminating defects, non-conformances and the need
for remedial work. Contractor shall fully adhere to quality assurance and inspection requirements as
specified detail in Exhibit L – Quality Management / General Requirements of the Contract.

Contractor shall ensure its quality management system conforms to the principles of an internationally
recognized standards such as the SNI/ ISO 9001; 2008, API Spec. Q1 or equivalent internationally
recognized Quality Management System Proper quality management system shall be fully implemented
in the following activities but not limited to:
1 Project management of the Work;
2 Planning of every element of the Work during each phase of the Work;

3 Execution of every element of the Work during each phase of the Work;
4 Verification that the Work is performed as per Contract requirements;
5 Supply chain management including supply of quality materials and equipment as well as material
management and control; and
6 Quality walkthrough, Quality Observation Card, and Quality audit to ensure the quality objectives
are fulfilled.

2.1.1.5.2 Quality Functions

Contractor’s quality management of the Work shall include all quality-related functions at all sites,
including:
1 Quality assurance;
2 Quality control;
3 Quality surveillance and inspection;
4 Engage and manage a third-party quality Subcontractor, as required by Company and Indonesian
Government regulations; and
5 Quality awareness training to ensure quality procedures and requirements are understood by all of
Contractor’s personnel.

2.1.1.5.3 Quality Actions

Prior to the commencement of Work, and before any new phase of Work is undertaken, Contractor shall
ensure through its quality management system that:
1 All applicable systems, procedures, specifications, and documentations are in place, and all
personnel are fully familiar and in compliance with them;
2 Project documents register is established and maintained/updated;
3 All correct engineering data are available including the latest revision of applicable documents,
drawings, specifications, codes and other pertinent documentations;
4 Company has approved the detailed design, including signing off on relevant documents and
drawings;
5 All required testing and qualification of personnel and work processes, and all other pre-requisites
for Work to be allowed to start have been duly carried out;
6 Correct and fully certified materials, together with positive material identification for alloy
material (if applicable) and all required resources are available; and

7 All plant, equipment, and facilities are fit for purpose, certified and calibrated as required and
suitable in every respect for the safe and satisfactory performance of the Work.

2.1.1.5.4 Quality Plan

Contractor shall prepare and submit for Company’s approval a Project Quality Plan within fourteen (14)
calendar days from the Effective Date. Project Quality Plan shall describe Contractor’s quality
management system in accordance with the requirements of ISO 9001;2008. It shall show, in logical
sequence, how to manage quality requirements for all activities performed by Contractor including
Subcontractors and Suppliers.

Project Quality Plan (PQP) shall describe the requirements/specifications for all activities performed by
Contractor including Subcontractors and Suppliers. During the period between Effective Date and
finalization of Project Quality Plan and Project Execution Plan, Contractor shall manage and perform all
aspects of the Work in compliance with Company specifications and procedures as listed in Exhibit G –
Company Supplied Data.

2.1.1.5.5 Subcontractor

Only one layer of subcontracting is permitted. Contractor shall be responsible to ensure that the selected
Subcontractor not subcontract the Work to other party for whatever reason. If Company finds that
Contractor breaches this rule, Company has right to take the following actions:
1 Stop the works of the Subcontractor immediately;
2 Assign third party to perform the Work that has been subcontracted by the Contractor;
3 All cost occurred to Company will be back charged to Contractor account; and
4 Any schedule delay occurs due to this clause shall be at Contractor’s full responsibility.

Contractor shall manage all Subcontractors to ensure fulfilment of all quality requirements as stated in
Exhibit L – Quality management / General Requirements. Contractor shall plan and conduct periodical
quality audits on Subcontractors in accordance with approved quality plan. Sufficient notification to
carry out Subcontractor audit shall be given to Company to allow Company witness or participate in
such audits. Notification shall be given in at least fourteen (14) calendar days in advance of the planned
audit date in Indonesia or thirty (30) calendar days in overseas.

2.1.1.5.6 Supplier of Materials and Equipment

In conjunction with supply of materials and equipment to Company, Contractor shall:


1 As required, audit Candidate Suppliers to verify their financial, organizational, technical and quality
capability and their level of experience;
2 Review all Candidate Suppliers’ documentation for compliance with Contract quality requirements;
3 After Contractor’s internal review, submit the Supplier’s documentation, along with Contractor’s
own internal marked up Inter Discipline Check (IDC) form, to Company for its review/comment;
4 Ensure the Suppliers’ quality documentation is approved prior to commencing the Work;
5 Review and endorse the criticality assessment and determine the level of quality surveillance
required in order to ensure the technical integrity of the product or service;
6 Manage all Suppliers to ensure their proper quality performance in accordance with Contractor’s
responsibilities under the Contract;
7 Conduct quality audits of Suppliers in accordance with Contractor’s audit plan. Sufficient
notification of Contractor’s intent to carry out Supplier audits shall be given to Company, to allow
Company’s witness of or participation in audits;
8 Ensure that all Factory Acceptance Tests (FAT) and Final Inspections are witnessed by duly
qualified Contractor inspectors, and Company’s Certification Contractor, MIGAS, SKKMIGAS and
Company Representatives as required; and
9 Issue the periodic reports to Company of all planned and performed quality audits. Contractor shall
promptly provide any supplementary information requested by Company in relation to Contractor’s
audit and verification activities.

2.1.1.5.7 Quality Audit/Inspection

Contractor shall plan and carry out a systematic program of audit and inspection activities Contractor
shall establish a schedule for formal audit/inspection of its own processes, procedures, materials,
equipment, tools, and personnel related to the Work during the engineering, procurement, fabrication,
construction, tie-in, installation, pre-commissioning, and commissioning/start-up phase. Contractor shall
seek concurrence from Company of its proposed audit/inspection schedule, which shall be submitted to
Company in a timely manner.

Company shall have an option to participate in any such audit/inspection or to perform a joint audit,
excluding any audit or part of audit that may be of a proprietary nature to Contractor. Any non-

conformances and corrective actions identified from any review audit or as may otherwise be identified
shall be promptly rectified by Contractor. The mitigation action to rectify fault shall also subject to
Company approval prior to be implemented.

For any type of inspection required to be witnessed by Company as specified in approved Project
Quality Plan (PQP) and Inspection & Test Plans (ITP),

1 Contractor shall provide a written invitation to request an inspection or test no later than ten (10)
calendar days prior to date of inspection in Indonesia or thirty (30) calendar days prior to date of
inspection in overseas which doesn’t require Government presence. Should the ITP indicate that
government authority representative such as Disnaker, DIMET, MIGAS and/or SKK MIGAS
should witness and/or approve a test or an activity, Contractor shall initially notify Company at least
30 calendar days in advance of the planned test date for items made in Indonesia and 60 calendar
days for the items produced in overseas, with an update on status every two weeks thereafter.
Company will not accept any request for inspection or test less than the given timeframe or changes
of schedule in short notice for whatever reason;
2 If Contractor fails to make ready matters to be inspected or any test required to be witnessed by
Company as specified in the approved Inspection & Test Plans (ITP) according to the confirmed
schedule, Contractor shall bear all incurred costs related to the inspection;
3 Re-verification test shall only be given for once. Contractor shall bear all incurred cost for any
additional re-verification test required to be witnessed by Company as specified in the approved
Inspection & Test Plans (ITP);
4 Any inspection or test required to be witnessed by Company as specified in approved Inspection
and Test Plan (ITP) shall be held in the Contractor/Subcontractor/Supplier Workshop stated in the
written invitation;
5 Any delay to the Work due to rejection to the inspection result shall be responsibility of Contractor.
There will not be any additional cost or schedule will be granted by Company as the result of this
clause; and
6 If Contractor performs, continues or closes the Work without any inspection or test witnessed by
Company as specified in approved Inspection and Test Plan (ITP), Contractor shall demolish or
rework the Work. There will not be any additional cost or schedule extension will be granted by
Company as the result of this clause.

2.1.1.5.8 Non Conformance Report

Company have right to issue Non Conformance Report (NCR) upon any quality non-conformity
discovered. Necessary corrective action plan shall be identified clearly by Contractor and shall be
submitted for Company’s approval within forty-eight (48) hours after NCR issued.

Once corrective action plan has been approved by Company, Contractor shall complete the corrective
action as per plan within ten (10) business days. If within that period Contractor cannot complete the
corrective action, Company shall have right to:
1 Stop Contractor’s works within areas as specified in the NCR;
2 Hold any progress payment for the respective month; and
3 Take over the corrective action or assign other contractor to complete the corrective action. All
costs occurred to Company in performing/ completing the corrective action shall be back charged to
Contractor. Takeover of corrective action by Company or by other contractor shall not release
Contractor from required responsibilities as per Contract.

2.1.1.5.9 Minimum Levels of QA/QC Personnel and Inspection Plan

Without relieving Contractor of any of its obligations to perform QA/QC services as defined elsewhere
in the Contract, Contractor shall provide as a minimum the numbers of quality personnel at the
indicated locations as defined in Exhibit L – Quality Management / General Requirements. All
personnel will have suitable qualifications and experience for the work to be performed. Duration of
assignment shall be as necessary to carry out all QA/QC duties, and not less than the durations
stipulated below.

Contractor’s overall QA/QC organization, duration and manning shall be agreed between Company and
Contractor prior to commencement of manufacturing and fabrication activities. Contractor shall
generate Inspection and Test Plan that cover all phase of the works which fall under its scope as stated
in the contract. Contractor shall give advance notification within agreed time to Company quality
personnel prior to carry out inspection and quality surveillance.

2.1.1.5.10 Quality Recognition

Contractor shall have the program in the Quality recognition as part of the Project scope to educate the
team of the awareness of the QA/QC. The recognition shall be in regular basis and incidental basis at
site and fabrication to the member of team that committed with the quality. The recognition also shall be
conducted on the major QA/QC milestone event of the project, including completion of the project as
the biggest milestone.

2.1.1.6 Regulations, Certification, and Permitting

2.1.1.6.1 General

Contractor shall be responsible for determining and complying with the requirements of government
authorities having jurisdiction over the Work as related to Contractor’s scope and acting there under,
complete certification, and permitting requirements affecting the performance of the Work. The scope
includes the areas of health, safety, environment, equipment and facilities certification/permit
requirements to be obtained from the relevant authorities.

Contractor shall prepare and issue a register identifying the current (or otherwise specifically
applicable) version of all documents containing regulatory, certification, and permitting requirements
applicable to design, engineering, procurement, and transportation, construction, installation, tie-in, pre-
commissioning and commissioning/start-up. The register shall also identify when those documents will
be required for each phase of the Work.

Contractor shall keep the register fully updated at all times throughout the performance of the Work.
The register shall identify the latest editions of such documents and where each document can be
accessed freely by Contractor’s and Company’s personnel. Note that the editions of these documents
that are current at the time of Contract signing will be the basis of design, procurement and
construction. Updates and later editions that are issued during the course of the Contract are to be
reviewed by Contactor and the effects on the Work described to Company for evaluation.

All materials and equipment supplied by Contractor including but not limited to side boom, crane,
welding machine, backhoe loader, grader, excavator, low boy, trucks, air compressor, pump, generator
set, cable drum rolling support, insulation resistance tester, mandrel pipe bend and metal scan detector,

cable detector, and gas detector shall be fit for purpose with valid certification, and subject to Company
inspection. The equipment that passed inspection shall be labeled or identified with proper manner for
easy monitoring purpose.

Contractor shall be responsible for securing all required certifications, permits, licenses, permissions for
construction works. Contractor shall be responsible to obtain the relevant government certificate for all
equipment, and shall be responsible to maintain such certification while they are being used at site.
Equipment which is used on the site shall be in good condition and certified by government/the
approved third party appointed.

Any crane operator shall have SIO MIGAS certification. Any lifting rigger shall have MIGAS
certification at minimum. Contractor shall provide dedicated certified scaffolder inspector (back to
back) and certified scaffolders for construction work.

Issues to be resolved related with Regulations, Certifications, Classifications, Permitting, Licenses,


Permissions shall be listed in Interface Register and Contractor shall be responsible to obtain those
certificates or permission as mentioned in scope of work of the contract. Contractor shall provide
assistance for Equipment/Installation Certification as requested by Company. Any impact due to delay
in obtaining approvals, certificates, permits and the like shall be at Contractor’s own cost and
responsibility.

2.1.1.6.2 Regulations

Contractor shall be familiar and comply with Indonesia government regulations wherever applicable
that apply to the scope of work of this Contract. In any contact with government authorities or their
certifications agencies, Contractor shall make any representation on behalf of Company.

2.1.1.6.3 Certifications

Project Quality Plan (“PQP”) shall identify which equipment and/or unit requires certification as per
Inspection and Test Plan (“ITP”). The ITP shall be updated prior to fabrication and manufacturing
commencing to indicate where any Governmental Authority Representative is required to witness
and/or approve a test or an activity. Contractor shall be responsible for the following items but not
limited to:

1 Contractor shall be responsible for obtaining all certifications required for complying with
Indonesian Government regulations. This shall include MIGAS certification required for new
equipment/units such as Pressure Vessel and Pressure Safety Valve;
2 should the Inspection and Test Plan (ITP) indicate that Government Authority Representative such
as MIGAS and/or SKKMIGAS should witness and/or approve a test or an activity, Contractor shall
initially notify Company at least thirty (30) calendar days in advance of the planned test date for
items made in Indonesia and sixty (60) calendar days for the items produced in overseas, with an
update on status every one week thereafter;
3 securing certification and acceptance for Contractor’s and or Sub-contractor’s welders, welder
qualification tests, welding specifications and welding procedures; and
4 securing certification or licenses for Contractor’s construction equipment (including the operators)
and lifting devices used during execution of the Work.

2.1.1.6.4 Permits

Contractor is responsible for obtaining all permits, documents, and licenses required for performing the
Work. Those include but not limited to visas and work permits for personnel; licenses and the like for
importation of materials and equipment into Indonesia. Any consequences for non-compliance by
Contractor in respect of the required permits, documents, and licenses to perform the Work shall be to
Contractor’s sole responsibility including payment of fines.

2.1.1.7 Community, Relations, and Securities

2.1.1.7.1 General

In relation to this Section, the following shall apply:


1 All Contractor’s Company Social Responsibility (“CSR”) and Community Relation activities, and
Security at Company Work Site shall be in line with Company policies (such as Business Ethics and
Conduct as defined in Exhibit O), and overall Company Community Development Program in the
respective areas, seeking guidance from and coordinated with Company Community Relation and
Security Representatives, and approved in advance by Company’s Project Manager. Contractor
shall implement all guidance provided by Company representatives. Any risks involved due to
failure to meet any of these shall be at Contractor’s own cost and responsibility;

2 In the event Company’s approval, concurrence, acknowledgement and/or coordination is required,


Contractor shall submit its request, information, and/or data, in writing to Company by not later
than sixty (60) calendar days prior to its planned activities;
3 Any risks involved due to delay in providing the request, information, and/or data to Company
which could resulted in delay on Company’s approval, concurrence, acknowledgement and/or set-
up coordination with related parties shall be at Contractor’s own cost and responsibility, and shall
not relieve Contractor’s obligation to complete the Work in accordance with the Contract Schedule
without any additional cost to Company;
4 Disruption of work shall mean any activities surrounding the work area by community that prevent
Contractor to execute the Work. In this event, Contractor shall work closely with Company in
resolving the issues;
5 Contractor shall meet Company’s “Zero” target for work disruption due to CSR, Community
Relation activities, and/or Security issues; and
6 Any disruption to the Work or if Contractor sees there is potential work disruption due to CSR,
Community Relation activities, and/or Security issues shall be informed and communicated by the
Contractor CR Representative to the Company CR Representative within 1 (one) hour after each
occurrence. In such event, Contractor CR Representative shall follow and implement guidance
made by the Company CR Representative. Any risks involved due to failure to adhere Company CR
Representative’s guidance shall be at Contractor’s own risk and responsibilities including any
standby of personnel and equipment.

2.1.1.7.2 Community Relation

1 Contractor shall be cognizant, at all times, of the importance of maintaining good community
relations, particularly in areas in the vicinity of the Work Site. To this end, Contractor shall
establish, and thereafter implement, the policies and procedures necessary to maintain good
community relations;
2 Contractor shall appoint and have available a Community Relations Representative (“Contractor CR
Representative”) by no later than twenty (20) calendar days after the Effective Date. Contractor CR
Representative must be available at the Work Sites at all times during the execution of the work at
site, starting from initial mobilization of Contractor’s resources up to completion of the Work;
3 Company will appoint a representative in all community relations and Company Social
Responsibility (“CSR”) issues (“Company CR Representative”);

4 prior to conducting activities at Company Work Sites, Contractor shall conduct pre-survey at the
Company Work Site to obtain photos/pictures and data of the actual existing condition of roads,
buildings, plants, ponds, jetties, and bridges (“Original Condition”) including any extended area that
might be required by Contractor in performing the work. Prior to the pre-survey, Contractor shall
inform and coordinate with Company CR Representative;
5 Company will conduct regular meetings with local community representatives and/or committees to
keep them informed of the activities which may affect the local community and to resolve, in an
equitable manner, concerns or issues raised by such representatives and/or committees. Contractor
CR Representative shall participate and support Company in such meetings, and shall implement
any meeting result that requires Contractor to take action;
6 Contractor shall obtain Company’s Project Manager’s approval of any modifications to the
activities in order to resolve such concerns or issues;
7 issues to be resolved related with Community Relations shall be listed by Contractor CR
Representative in Interface Register as different section;
8 Company CR Representative will take lead in all interfaces with surrounding community during on
site activity. Contractor shall participate and support Company in the interfaces activities. Any risk
involved due to failure to implement Company guidance, including, but not limited to, work delay
or personnel standby or equipment standby shall be at Contractor’s own cost and responsibility;
9 Company requires Contractor to use local labor (skilled/unskilled) from surrounding community
and villages, located in the same area (Desa/Dusun) with Company facilities;
10 prior to hiring the local labor, Contractor shall inform Company CR Representative. Company CR
Representative shall facilitate the selection with the surrounding community;
11 any damage due to, directly or indirectly, Contractor activities at Company Work Site, including but
not limited to public and/or private road access to/from Company Work Sites, bridge, jetties, public
and/or private properties, etc. shall be repaired and restored by Contractor to the Original Condition
promptly, at Contractor’s own cost and responsibility. In such event, Contractor shall inform and
coordinate with Company CR Representative in advance; and
12 Contractor shall provide road repair permit which shall be coordinated with local government and
other related parties. Contractor shall assign community relation officer to coordinate with
Company Personnel to obtain any permits as required.

2.1.2 Temporary Facilities and Services for Company Personnel

2.1.2.1 Temporary Facilities

Facilities and services for Company personnel in Contractor’s Engineering Office and Company Work
Sites shall be provided by Contractor, with details as specified in Exhibits F. Refer to Exhibit F of the
Contract. Contractor shall include cost of providing such facilities and services in the Contract Price.
Temporary facilities and services requirements are part of lump sum value.

2.1.2.1.1 Jakarta Office

Contractor shall provide office space of dedicated meeting room completed with the meeting table and
ergonomic chairs for 20 personnel for Company’s Representative in Jabotabek. The facility shall
include computer system, internet with minimum 10 MB capacity, Multi-Function Device (“MDF”),
stationary, daily drinking water, coffee, meals and other refreshment, in-focus. Contractor shall provide
the Office Boy service as the support.

2.1.2.1.2 Office at Contractor’s Workshop

Contractor shall provide office space for two (2) Company Representatives desk including computer,
internet with the capacity 10 MB, stationary, MDF, daily drinking water, coffee, meals and other
refreshment.

3.1.2.1.3 Site Office

Contractor shall provide office space for Company’s Project Team for maximum ten (10) personnel at
Site with detail requirements as specified in Exhibit F – Administration. The Company site office is
located in the east part of Company’s Grissik plant fence.

2.1.2.2 Services

Contractor shall responsible for service the Company Furnish Material


supervision/expert/integration/termination personnel of transportation and accommodation at site.
Contractor shall maintain that the accommodation as per Company standard. The estimate duration of
Company Furnish Materials services of 3 months basis for maximum 5 personnel. Any extension of the
stay duration due to Contractor failure or construction progress delay are part of Contractor
responsibilities. Contractor shall support all the equipment, tools, materials, PPE, stationary, printing,
etc. for the supervision, termination, integration, testing, calibration, etc.

2.2 Detail Engineering, As Built and Final Documentation

2.2.1 General

Contractor shall responsible to review, assess, verify, validate, and satisfy itself of the Company
previous engineering work including Exhibit G – Company Supplied Data. Company shall have no
responsibility in the event of any discrepancy/inconsistency of the data and information provided.
Contractor shall conduct site survey and any other activities required for verifying previous engineering
work and obtain all information that may be necessary to prepare the Approved for Construction/Issued
for Use engineering deliverables.

Contractor shall perform all engineering activities necessary to provide Approved for
Construction/Issued for Use (AFC/IFU) design documents and drawings to be used during fabrication,
construction, installation, tie-in, pre-commissioning, and commissioning/start-up based on Company
Supplied Data.

Contractor shall identify all engineering deliverables, in total and for each discipline, and compile a
complete deliverables list within fourteen (14) calendar days from the Effective Date. This deliverables
list shall be attached in Engineering Management Plan. Contractor shall diligently check all Company
Supplied Data for inconsistencies, and advise Company of same.

Contractor shall complete the engineering review and verification of the documents and drawings
contained within Company Supplied Data and advise Company of any identified errors. Contractor shall
provide all documents necessary to certify the design of the Work and prepare all As-Built documents
and drawings to complete the certification of the installed Work to Company’s and Governmental
Authorities’ satisfaction.

The design, the execution and the completed Work shall comply with any and all applicable
specifications, technical standards, building, construction and environmental codes specified by
Company or defined by any applicable Indonesia Government laws and regulations as well as
international codes and standards. (Shall be use the latest update with minimum 1 years before
engineering design (if any).

A list of different design codes, specification and design manuals, which will be referenced to, but shall
not necessarily be limited to, is presented in the latest revision of Document No. ID-N-CG-PP0-BOD-
PL-25-0003 – Basis of Design in Exhibit G – Company Supplied Data.

2.2.2 Detailed Engineering

2.2.2.1 Process & Safety Engineering

Health, safety, and safeguarding of the environment are Company core values. Contractor shall take
into consideration health, safety and the environment throughout execution of the Work. Process
engineering efforts required by Contractor shall include but not be limited to verifying and updating
process documents and drawings provided in in Exhibit G – Company Supplied Data. Contractor shall
consider all aspects of process design and engineering to facilitate fabrication, installation, tie-in, pre-
commissioning, and commissioning/start-up as well as Safety in operability and environmental impact
of the facilities in accordance with design criteria and functional requirements.

Contractor shall update Process simulations and/or Heat and Material Balance (HMB). Process
simulation shall be prepared using HYSYS Minimum version 7.1 or 7.3 or other Company approved
simulation program. Simulation runs shall be made at any time an update occurs or a modification is
proposed during the detailed design that may affect the process facilities, in results of any process
updates shall be carried through the design as required.

Contractor shall check, optimize and confirm all process, utilities systems to meet the work
performances and functional requirements. Process and safety detail engineering deliverables shall
include, but not be limited to:
1 Process Flow Diagram (“PFD”) and Piping and Instrumentation Diagrams (“P&ID”);
2 process datasheet;
3 Line sizing and required hydraulic calculation;

emergency shutdown philosophy and procedure; and
13 Acceptance procedure and report.

Safety engineering deliverables shall include, but not be limited to Hazard Identification (HAZID), and
Hazard Operability (HAZOP) workshops and reports, as amendments to those already completed by
Company and provided in Company Supplied Data and validation of other safety studies.

2.2.2.2 Civil & Structure Engineering

Contractor shall carry out all necessary detailed civil discipline design and engineering including
procurement and construction support. Civil & Structure Detailed Engineering deliverables shall
include, but not limited to:
1 Construction procedures for earthwork, concrete, and structural works;
2 Composite lay-out of drainage and underground utilities;
3 Detailed drawings of drainage and underground utilities;
4 Foundation location plan/key plan;
5 Detailed drawings of equipment foundations and modification of existing equipment foundations;
6 Detailed drawings of equipment structures foundations;
7 Detailed drawings of ultrasonic metering shelter;
8 Detailed drawings of modification existing pipe rack foundation;
9 Detailed drawings of pipe support and miscellaneous foundations;

Contractor shall carry out all detailed piping discipline design and engineering including procurement
and construction support. Piping Detail Engineering deliverables shall include, but not be limited to:
1 line list (e.g. Line list showing piping class, size, process conditions, insulation and pressure test
requirements, stress criteria, NDE (radiography, hydrotest, pneumatic and leak test) for each line);
2 Valve data sheet;
3 Specialty Piping (SP) items data sheet;
4 Spare part list for valve and SP items;
5 Tie-in schedule;
6 plot plan & key plan;
7 piping plan (e.g. piping plans, 3D model, sections and elevations, piping route, location of valves

Contractor shall refer to the following documents for the execution of piping detail design engineering:

The pipe stress analysis shall confirm adequate support, guides and anchors are provided to limit
thermal and pressure induced stresses and equipment nozzle loads to allowable levels. Dynamic
response analysis shall be performed as necessary to ensure resonance does not occur for piping
connected to equipment. All calculation shall be updated to incorporate Company Supplied Data.

The pipe stress report shall be submitted for Company review and approval during detail engineering.
Contractor shall conduct tie-in surveys to determine the actual elevation, coordinates, flange orientation,
(flange tilt/ inclination and bolt hole rotation) of the tie-in points. The tie-in survey data shall be used as
a basis for the development of tie-in pipe spools fabrication isometric drawings.

The piping layout drawings shall define general pipe routing and equipment layout to show equipment
clearance and access provision. Piping layout drawings shall incorporate actual Company Furnished
Material data. Contractor shall ensure pipe and cable corridors are identified early in the design to avoid
pipe, cable tray, and structure clashes.

Contractor shall prepare piping plan, section, and pipe support detail drawings for all equipment and
piping, with sufficient detail to define pipe routing, pipe support location, slope, valve location and
orientation, instrumentation, control device installation, show equipment clearance and access
provision. Drawings shall incorporate actual purchased equipment data.

Piping Isometric shall be detailed, sufficient and can be used for procurement and fabrication. It shall
also include the design conditions, testing conditions, support type and location, dimensions, material
take-off (including piping component size, number of material and material of piping component).
Contractor shall prepare isometric spool shop/fabrication drawings to expedite the fabrication Work
process, material control, and piping spool installation. The isometric drawings shall be used as
reference of preparing the welding map and NDT of each joint thereafter. Contractor shall develop and
finalize the isometric drawing during detail design.

Contractor shall design piping flange and bolting details to accommodate hydraulic bolt
tensioning/torque requirements in accordance to Company’s Flange Management Procedure. Hydraulic
bolt tensioning/torque shall be used on all connections. Contractor shall provide pipe support and
installation details for review and approval by Company.

Contractor shall prepare detailed procedures for hydrostatic testing to be carried out in Grissik site
followed by flushing/blowing, leak testing and purging. The procedures shall include P&IDs showing
blinds, vents, drains and fill connections, test pressures, testing media, test zone boundaries and pipe test
legend. The procedures shall detail instrument removal and reinstallation, recording instrument
locations and requirements, purge and Nitrogen gas volumes and pressures, valve operating positions,
identification and sequences, and a detailed execution schedule. Provision of consumables (water,
chemical, gasket etc.) and temporary blind flange for hydrostatic Work shall be under Contractor’s
responsibility.

Contractor shall complete all valve data sheets in conjunction with process and mechanical/ materials
engineering. Contractor shall prepare Valve Material Take Off identifying valves size, type, material
and quantity. All design requirements, such as general and special requirements, material specifications,
valve operation, valve type, body and internal design, auxiliary requirements, dimensions and end
connections shall comply with Company’s specifications, applicable codes and standards. Contractor
shall also implement a valve numbering system in accordance with Company’s specifications.

Contractor shall prepare layout drawings showing design layouts for all equipment packages. The layout
drawings shall be detailed drawings, suitable for fabrication, transportation and installation. The
maximum unencumbered clearance shall be considered in the equipment layout for removal of
equipment parts for all maintenance and overhaul activities. Contractor equipment layouts shall be
submitted to Company for review and approval. Layout drawings shall include layout of major

2.2.2.4 Instrument & Control Engineering

Contractor shall carry out all detailed instrumentation and control discipline design and engineering
including procurement and construction support. The instrumentation and control system shall be
designed to control and monitor all new equipment/facilities which are covered under this project by
modifying the existing Grissik control system facilities.

Contractor shall engage and manage Control System Supplier (DCS and SIS) appointed by Company to
perform the required modification. Selection of material and supply of all field instrumentation and
equipment shall refer to the latest revision of Document No. ID-G-BU-GG0-AML-GN-00-0001 –
Approved Vendor List. Selection and installation of field instrumentation shall refer to the latest
revision of the following Document:

including the experience technician.

The entire cables from new instruments shall run to the new Junction Box. All the multi pair cores
cables from new Junction Box shall run to the existing Power Control Room (PCR-A and SSWJ
Metering Room) thru cable tray. Contractor to provide new cable tray from metering skid area to PCR-
A and/or SSWJ Metering room/CCR. Contractor shall provide new Junction Box outside the metering
skid. Analog signal, digital signal, and solenoid shall be separated.

2.2.2.5 Electrical Engineering

Contractor shall carry out all necessary detailed electrical discipline design and engineering including
procurement and construction support. Electrical Detail Engineering deliverables shall include, but not
be limited to:

2.2.2.5.1 Power System

Contractor shall modify the existing Normal Lighting and Small Power Distribution Board Single Line
Diagram due to potential additional normal and emergency lighting fixtures for this new facility.
Contractor shall carry out site visit to check availability of spares and space breakers. Contractor shall
review and update the existing electrical cable schedule for power, control and lighting. Contractor shall
perform electrical cable sizing to determine adequate size and voltage drop as basis to develop cable
schedule. Contractor shall carry out site visit to check cable route, cable length and additional cable
tray/trench if required. Contractor shall feed power supply to new metering panel and metering devices
from existing UPS system at Grissik plant.

2.2.2.5.2 Area Classification

Contractor shall update hazardous area classification drawing for Grissik Plant to accommodate the
additional facilities based on API RP 500 “Recommended Practice for Classification of Location for
Electrical Installations at Petroleum Facilities Classified as Class I, Division 1, and Division 2. The
Contractor shall provide the hazardous equipment list as basis to develop hazardous area classification
drawing.

Selection of equipment for Installation in any particular area shall be to API-RP-500. In the interest of
standardization it may be the case that equipment installed in a particular zone may be certified for used
in a hazardous location. Electrical Apparatus shall be certified for use in one of the specific hazardous
areas classified as follows:

Gas Group D short time

This area classification deals only with risks due to flammable gases and vapors and by implication,
flammable mists. All electrical equipment installed in the process plant shall as a minimum be certified
for use in a Class I Division 2 Area. The degree of environmental protection for equipment for use in
hazardous area shall be minimum NEMA-4X and temperature classification T3 (200 degree Celsius).
Equipment selected for use in accordance with NFPA requirements shall be certified by Underwriters
Laboratories Inc. (UL) or equivalent. Test certificates approved by the above certifying authority shall
be obtained for each item of certified equipment. Markings on nameplates shall conform to certification
requirements.

2.2.2.5.3 Lightning Protection and Grounding

Lightning protection and grounding layout drawings shall show all required grounding and bonding.
Plan views shall show major equipment and structure in the background with text describing equipment
being grounded.

2.2.3 As-Built and Final Documentation

Contractor’s Contract obligations shall not be considered fulfilled until as-built is completed to the
satisfaction of Company. Contractor shall, at the completion of engineering activities, prepare an As-
built Register of all project documents and drawings that require As-Built and/or update for the scope of
work.

Contractor shall provide and manage all As-Built engineering activities after all fabrication and
construction activities completed including Company Furnished Materials. Contractor shall designate
from its project management organization an engineering focal point that will be responsible for As-
Built activities. The focal point shall be appointed before commencement of fabrication and
construction and shall coordinate all As-Built engineering activities until completion to Company’s
satisfaction.

Notwithstanding the responsibility of Contractor to ensure As-Built of Supplier’s and Subcontractor’s


documents, drawings, and data, Contractor shall be responsible for ensuring that all As-Built
Supplier/Subcontractor data is captured in Contractor’s As-Built documents, drawings, and data. There
shall be no missing information or ‘holds’ on the As-Built documents and drawings. Contractor shall
prepare final documentation including.

Receiving, reviewing and compilation assembly final data from all Suppliers or Subcontractors; and
Updating and compiling all detailed design documents and drawings at As-Built status based on marked
up drawings provided by Contractor’s fabrication, installation and survey teams and from marked up
drawings provided by any Subcontractors as applicable.

Contractor shall provide final documentation including but not limited to:
1 Engineering deliverables including native electronic and pdf files format;
2 Fabrication deliverables;
3 As-built documents and drawings (to be agreed between Company and Contractor);
4 Electronic files, including calculations, analyses document/drawing registers and engineering
registers;
5 Contract HSE Plans and Reports;
6 Contract QA/QC Plans and Reports;
7 Manufacturing data books;
8 Procurement-related data;
9 Packing and shipping records;
10 Fabrication dossiers;
11 Installation dossiers;
12 Hydrostatic test dossiers/records;
13 Inspection and certification dossiers;
14 Pre-commissioning dossiers;
15 Commissioning dossiers;
16 Change Order control records;
17 Contract specifications and procedures; and
18 All certifications from Third Party inspectors and Governmental Authorities.

All documentations that will be handed over to Company shall be in accordance with the below

2.3 Procurement

2.3.1 General

Contractor shall provide all resources to procure, expedite, inspect, and transport all materials, and
equipment as necessary for completion of the Work in accordance with the Contract. Contractor shall
prioritized detail engineering deliverables to support critical procurement item. With respect to
procurement and materials activities, Contractor shall be responsible for the following:
1 Prepare a procurement schedule featuring all packages and services as required by Company
including forecast, planned, and actual dates for all key activities;
2 Maintain an efficient material management system providing full traceability through of the
materials and equipment and their specific movements and locations;
3 Ensure that the terms and conditions of purchase/subcontract and scope of supply/ service/work
include the requirements as set out in the Contract;
4 Expedite all items such as Supplier technical and design data for approval and use by Contractor’s
engineering and for Company’s data collection tool;
5 Arrange for and expedite delivery of all materials and equipment, including Company Furnished
Material, to site;

6 Arrange custody transfer of Company Furnished Material to Contractor warehouse/storage prior to


installation;
7 Arrange for the timely visits of Supplier Representatives as required by Company
8 Prepare and implement an inspection plan for inspecting materials and equipment at receiving,
dispensing, and “after installation”;
9 Transfer of all warranties and guarantees, operating manuals, maintenance manuals, Supplier
Document Control Index and the like to Company for all procured materials and equipment;
10 Arrange for Company’s and Company’s third party inspection and testing as required by Company
11 Arrange and obtain any necessary permit and approval required to procure up to delivery and
receiving of the materials and equipment to designated location;
12 Provide handling of the material and equipment at each of point of delivery (departure and arrival);
13 Provide reporting material status (issuing and surplus) including but not limited to Material
Receiving Report (MRR) after material received at Site for all material which shall be signed by
Company representative;
14 Conduct joint inspection with Company during material receiving at Site prior to storage and
preservation; and
15 Provide material handling, storing, and transporting Company Furnished Material from its supply
point or Company’s designated location to and at the Work Site. Contractor shall ensure proper
maintenance as well as preservation of the Company Furnished Material. Contractor shall, at its
own cost and responsibility, repair or replace any damage of the Company Furnished Material
resulting from any reason, while in the custody of Contractor.

All materials and equipment purchased by Contractor shall be brand new and must meet or exceed
Company’s technical requirements and specification, and certified in accordance with Company’s
requirement.

Company shall have the right to inspect and test all Contractor-procured materials and equipment and to
reject any item not in compliance with the specified documents and Company approved designs or fail
to pass the required test. Contractor shall arrange to replace rejected materials/equipment without any
additional cost to Company. Such replacement shall be performed in compliance with all applicable
requirements of this Contract, and Company shall have no obligation to grant an extension to the agreed
Contract Schedule and additional cost in respect thereof.

Procurement of any materials and equipment shall refer to Company’s AVL (Approved Vendor List),
and Company shall have right to reject materials which source from the unapproved manufacturer.
Procurement of any materials and equipment which are not listed in Company’s AVL (Approved
Vendor List) shall subject to Company’s review and approval by using TQ (Technical Query)
mechanism.

2.3.2 Company Furnished Material

Contractor shall note that certain long lead items will be procured by Company. The delivery points of
Company furnish materials of site Grissik free on truck. Contractor is responsible for offloading all
materials, storage, preservation, and all necessary service including security system. Contractor shall
done the offloading in such time that shall not be delayed. Any offloading delay and claim impact from
the Company Supplier will be born to the Contractor cost.

Company Furnished Material includes all materials listed in Exhibit J – Company Furnished Materials.
Ultrasonic Metering Package and the manual and actuated valve as Company Furnished Material will be
available in Grissik site within range of June 2017 to November 2017. Contractor shall responsibility
for services Company Supplied Material shall refer to Exhibit J – Company Furnished Materials.

For all Company Furnished Material, Contractor is responsible for conducting a visual check to detect
damage right after receiving at Supply Point witnessed by Supplier, Contractor and Company
Representatives. The visual check report shall be generated by Contractor per material Delivery. The
visual check report shall include at a minimum, quality of delivered material, date of delivery, condition
of material with related photos, and signed by Contractor Representatives and Company
Representatives. Contractor is responsible to ensure all dimensions and tie-in points meet as per
designed.

Contractor shall be responsible in storing, preservation and handling Company Furnished Material from
Company designated location to and at the Work Site. During handling and transportation at site,
Contractor shall maintain and ensure the material in a good condition and no damage. Any damage shall
be repaired or replaced by Contractor at Contractor’s own cost and responsibilities.
Contractor shall be responsible for demolish, installation of Company Furnished Material including all
tie-in piping, tubing and accessories, all termination, connection inside skid to outside of skid and vice
versa at Contractor’s own cost as part of lump sum.

Specific for provision of supporting SAT Work for Premium Delivery Valves, Contractor shall provide
testing and bunkering system (plate wall/container etc.) which sufficient for up to 20 inch valves.

2.3.3 Contractor Furnished Material

Contractor shall supply all materials and equipment necessary to complete the works. This includes, but
is not limited to:
1 Material required as part of the detail engineering, and not supply by Company (materials excluded
on the Company Furnished Materials at the Exhibit J – Company Furnish Materials.
2 Process safety materials of wheel type dry chemicals, potable dry chemicals, safety sign, new
escape route, new platform required for equipment sitting review results, safety signs, safety boards,
new eye wash and all connection and other process safety materials
3 Piping consumables, bulk materials, tools, equipment for fabrications, construction and installation
4 Instrumentation and Electricals cables, bulk, consumable, tubing, tools, equipment, etc
5 Instrumentation cable optics for data communication and accessories for connection metering
control room to the DCS/SIS.
6 Temporary office containers and all accessories
7 All materials required for supporting installation of ultrasonic metering package;
8 All materials required for civil and structural work (foundation, structural, shelters, etc)
9 All materials, bulk and consumable for fabrication work
10 All instrumentation bulk, consumable, software, tubing, fittings, and instrumentation work required.
11 All structural materials required for fabrication and site erection/installation (including vessel
platform extension, other access platform, ladder, handrail, and grating as required during detail
engineering stage);
12 All civil bulk materials required for construction of equipment foundations;
13 New fences for about 200 meter length and all accessories as per Company specifications
14 All necessary field instruments, instrument cables, cable trays, tubing & fittings, junction boxes;
15 All necessary electrical equipment (e.g. lighting fixtures), electrical and grounding cables, cable
trays, junction boxes, and other related accessories;
16 All necessary materials and equipment/tools required to perform construction, installation, tie-in,
pre-commissioning, and commissioning/start-up activities; and
17 All necessary consumables, lubricants, reagents, spares and other items to enable completion of
construction, installation, tie-in, pre-commissioning, and commissioning/start-up activities

18 Nitrogen required during commissioning/start-up of the scope of work covered volume for
continuous purging, leak testing, and the oxygen removal from the system as per Company approval
level. Contractor shall provide at least 5 rack nitrogen bottles (70 bottles) high purity nitrogen
including all the headers, tubing, 100 meter hose connection, pressure indicators, operators, and all
accessories required to execute the purging, leak testing, and all work during commissioning/start
up as part of the lump sum cost; and
Pre-commissioning, commissioning/star up, and performance test spare part

2.3.4 Pre-Commissioning/Commissioning Spare Parts & Special Tools

Contractor shall ensure procurement and delivery of all spare parts and special tools as required for pre-
commissioning and commissioning/start-up of the Work. Contractor shall identify and separately list all
material purchased as commissioning/start-up spares and make the list available for Company’s review
and approval. As a minimum, the commissioning/start-up spare parts shall include all recommended
commissioning/start-up spare parts informed by Suppliers. Contractor shall be responsible for providing
spare parts for the long lead items based on those consumed during commissioning/start-up activities.

2.3.5 Two Years Operational Spare Parts

Contractor shall submit to Company a list of recommended 2 (two) years operational spare parts for all
equipment, valves, instruments, and speciality items within the Company’s requested period.

2.3.6 Disposal of Surplus

Contractor shall be responsible for proper disposal of surplus equipment and materials which are not
supplied by Company. Contractor shall return the surplus equipment and materials which are supplied
by Company under direction from Company.

2.3.7 Export/Import

Contractor is solely responsible for making all necessary arrangements for the proper and timely
exportation from, importation into and re-exportation from any country of Contractor’s Supply, and the
Work, shall refer to Exhibit P– Procurement, Importation and Re-exportation. Contractor shall be
responsible for all applicable taxes, tariffs, levies, surcharges, fees and other charges. Contractor shall

do so in a timely manner to meet the Contact Schedule and in accordance with all Applicable Laws.
Upon deemed necessary, Contractor shall be responsible to air-freight the delay material to maintain the
contract schedule. Any errors, omissions or delays in, or resulting from, the import & re-export process
will not be grounds for a change.

Contractor shall also prepare for submission to the Indonesia Customs and/or other Government
Authority any reports required to account satisfactorily for the final disposition of all items imported
permanently or temporarily for the Work. Contractor shall be responsible for ensuring that all US
Export Compliance regulations and any other countries’ Export Compliance regulations are followed.

2.3.8 Packing and Preservation

Contractor shall be responsible for sufficient packing and protection of the materials and equipment for
shipment to the job site. Unless noted otherwise, the following factors shall be considered during
preparation for shipment:
1 No equipment or material shall be released for shipment without inspection by Company
representative. Inspection release note shall be signed by Company representative prior to any
shipment;
2 All slot mounted or plug-in instruments shall be removed and packed in their original protective
packing and shipped separately from the cabinets;
3 All equipment and material shall be properly crated to prevent damage during transportation,
handling and shipment. The preparation for shipment shall be subjected to the approval of the
Company’s inspector; and
4 Contractor shall wrap and tag all small items being shipped loose. All items shall be properly tagged
with the tag containing the item Tag Number, Purchase Order Number, Service and other relevant
details.

Contractor shall be responsible for preserving the materials and equipment arrived at the job site in
accordance to Company Document No. ID-N-BU-MA1-STS-GN-00-0002 - Preservation and
Packaging. Contractor shall also be responsible for preserving Company Furnished Material which have
been formally handed over (custody transfer) to Contractor. Company has a right to claim any damage
to Company Furnished Material after hand over/custody transfer and on which case Contractor shall
repair or replace it without any additional cost to Company.

Contractor shall identify, mark for identification, and properly store all materials. All stored materials
shall be supported off the ground (by wooden blocks or on shelving, etc.), shall not be standing in water,
and shall not be exposed to dusty or corrosive environments. Contractor shall provide on a 24 hours/day
basis secure fencing and security guards at site. Contractor shall provide weekly report of materials
received, in storage, issued to the fabrication site, lost, and damaged.

Contractor shall replace all lost or damaged materials. Damage, loss, theft, or corrosion of materials
occurring while in the custody of Contractor shall not be cause for delay in the project completion date.
Contractor shall be responsible for cost of damage, repairs, replacement, etc. of materials resulting from
negligence, accidents, improper storage, or any other reason, while in the custody of Contractor.

2.4 Fabrication, Construction and Installation

2.4.1 General

Contractor shall be fully responsible for providing qualified resources and managing all fabrication,
construction and installation activities in relation to the Work. Contractor’s project management
organization shall include an experienced Construction Manager in placed from the Effective Date. The
Construction Manager shall be specifically responsible for:
1 Optimization of overall construction plan/sequence, schedule, and resources (manpower,
equipment/tools and material) requirement referring to Contract Schedule;
2 Considering all hindrances including local community, security at Work location Company existing
operation condition, weather condition, road access, and target achievement respectively;
3 The verification of all construction, fabrication, demolition, installation and tie-in procedures;
4 Verification of welding procedures, welder qualifications;
5 Non-destructive testing requirements;
6 Development of resource loaded level 4 schedule in the latest version of Primavera for each
construction, fabrication, demolition, installation, tie-in, pre-commissioning and commissioning
activity;
7 Development of construction work pack (construction engineering, procedure, job safety analysis,
and other supporting data for obtaining Company permit to work) for each construction, fabrication,
demolition, installation and tie-in, pre-commissioning and commissioning activity;
8 Conducting constructability review and ensuring that the review findings are incorporated into
detailed construction procedure;

9 Defining acceptance criteria for completion of each construction, fabrication, installation and tie-in,
pre-commissioning and commissioning activity;
10 Ensuring complete work is in stick compliance with the approved design by Company; and
11 Construction recognition (site lunch or others) of the major milestone of complete shutdown work,
complete ultrasonic metering installation, mechanical completion, and completion
commissioning/start up for all the site team to maintain high performance construction work / site
activity.

Contractor shall provide all management team, manpower (certified, specialist, skilled and non-skilled),
accommodations, vehicles, equipment, tools, consumables and special items required to perform and
complete all the scope of works. All aforementioned items shall be comply with each Government
regulation and Company standards and specification appropriate with each disciplines (refer to Exhibit
G, Company Supplied Data).

Contractor shall responsible to complete all the scope of works stated in this contract. In doing so,
management, monitoring and check list tools and reports shall be provided and prepared systematically
through CCMS (Construction Completion Management System) as per ID-G-BU-YY0-PRO-GN-00-
0019_R1, “Construction Completion Management System Requirements”. Reports and certificates of
CCMS shall be approved by Company.

Contractor shall perform all construction general support, including but not limited to:
1 Monitoring and supervising construction activity to ensure achievement of approved project
schedule;
2 Perform and provide all construction engineering study required to perform all works;
3 Perform required inspection and test as per approved ITP (Inspection and Test Plan) in detailed
engineering phase for each discipline;
4 Temporary grounding system for all utilized equipment, tools and special items;
5 Scaffolding and its accessories erection, modification and demolition to support other piping, civil,
structure, instrument and electrical works;
6 Lifting and rigging works, plans, studies and executions process using appropriate manpower,
equipment, tools and accessories as per following Company standards and specifications:

2.4.1.1 Contractor Temporary Facilities and Services

Temporary site services shall include camp; lay down area/warehouse, preservation area and site
workshop. Contractor shall build an office and minor workshop/warehouse in Company operational
control demarcation. Design and implementation should be in accordance with the regulations and
approval of the Company.

Contractor shall make its own arrangements for the temporary power supply for all construction tools
and equipment, and for Contractors temporary office and facilities for the duration but not limited of all
on-site construction, installation, tie-in, pre-commissioning and commissioning/start-up activities.

Contractor shall supply, install, and maintain all temporary electrical installations for the construction
sites at its owned cost. Contractor shall provide its own water for both drinking and utilities. Contractor
shall maintain a healthy and safe environment at all sites. Contractor shall place high importance on
hygiene at the Contractors’ temporary facilities. Contractor shall remove all temporary facilities and
reinstate all works disturbed in thirty (30) calendar days after Provisional Acceptance Certificate
approved by Company.

Contractor shall provide the site office and the warehouse to store the Company Furnished Material and
Contractor Furnished Material. The location of site office and warehouse shall be within 15 km radius
from Company Grissik site. The detail of provided Contractor’s warehouse specification is defined in
document ID-G-BU-GG0-STR-GN-00-0013 – Minimum Requirement for Contractor Camp and Office.

2.4.1.2 Construction Management Plan

Contractor shall prepare and submit for Company’s approval a Construction Management Plan within
fourteen (14) calendar days from the Effective Date. Construction Management Plan shall at minimum
describe how Contractor manage all construction works including Work pack requirement,
Subcontractor requirement (if any), level 4 schedule, assignment for key construction personnel, and
resources loading to handle all construction works as per Contract Schedule.

2.4.1.3 Construction Engineering

Contractor shall perform detailed engineering for all activities required to support construction
methodology including temporary works and construction aids. Contractor shall complete all studies
and calculation as well as prepare all necessary documents and drawings to enable construction,
fabrication, installation including tie-ins, pre-commissioning, and commissioning/start-up of all items
under this Contract.

Contractor shall present preliminary fabrication plans, component handling and lifting schemes and the
like, to Company for review and approval, prior to the commencement of fabrication. Contractor shall

prepare construction documents in sufficient detail to satisfy all Authorities, provide suppliers and
construction personnel with sufficient instruction to execute, supply, erect, construct, commission,
perform acceptance tests and complete the Works and make good and remedy any defects, shrinkages
and other faults.

The analyses shall include material handling analysis, considering the dimensions and weights of all
equipment and considering the space limitations at the Grissik Station. All calculations, analysis and
design packages, construction documents, and other documentation shall be submitted to Company for
approval in a form of Construction Workpack. These documents have to be submitted prior to
commencement of site construction work.

2.4.1.4 Construction Workpack

Contractor shall prepare Construction Workpack for all key works including, but not limited to:
1 Pre-engineering survey;
2 Scan for underground pipelines, facilities, structures, cables and mark any identified by using
identifiable marker prior any installation activities.
3 Key construction activities (civil and structural, mechanical, piping, instrument, electrical);
4 Instrumentation calibration, continuity test, loop test, and function test;
5 Hydro testing, flushing and blowing; and
6 Nitrogen purging and leak testing, and other pre-commissioning activities.

Construction Work pack shall include the following details:


1 Relevant detail design documents and drawings;
2 Detail procedure to perform the work;
3 Job Safety Analysis (JSA) other supporting documentations for obtaining the work permit;
4 Site Risk Based Assessment (SRBA) for any hot work to be performed inside plant as per SIMOPS
Plan;
5 Resources list detailing required manpower classification as per HSE Training Matrix, as well as
required materials and tools/equipment to perform the work;
6 Qualification certificate of skilled manpower (e.g. welder, heavy equipment operator, rigger,
scaffolder, scaffolding inspector, etc.);
7 Construction plan and schedule;
8 Construction team/organization chart and each team member responsibilities;

10 Inspection and Test Plan (ITP); and
11 Material handling procedure.

Construction Work pack shall be submitted for Company’s review/approval prior to executing the
respective construction work.

2.4.1.5 Civil and Structure Construction Work

Contractor shall be responsible for the completion of site civil/structural works, including but not
limited to:
1 Conducting survey, prior and during the construction work;
2 Performing site preparation;
3 Conducting underground survey to detect any underground facilities that impacted by the
construction work;
4 Drainage system, including modification of existing open ditch and drainage pipes, and temporary
diversion of site drainage system required during the construction;
5 Underground facilities, including modification of existing;
6 Cable trenches including modification of existing;
7 Foundations for equipment, equipment structures, miscellaneous supports, access-ways, stairways
and ladders;
8 Construction, erection and installation of all structural elements for: pipe racks, equipment structure,
walkways, access platforms, pipe supports, stairways, ladders, handrails and grating;
9 Fire proofing for pipe racks, equipment structures and pipe supports as required in fire protection
study;
10 Site remediation; and
11 Remove the existing gravel to another area that will be determined by Company.

All Excavation on Company’s property requires Company’s prior approval with using daily permit.
Company will require hand/manual excavation of nearby underground facilities by Contractor prior to
allowing mechanical excavation. No schedule and cost impact for this work method.

2.4.1.6 Lifting and Rigging Plan

Contractor shall be responsible for the transfer of all equipment and materials from/to Grissik Station.
Valid certificate of all lifting and rigging equipment shall be provided for Company’s review and
approval. Contractor shall prepare a detailed installation lifting and rigging plan for all lifting and
rigging activities related to the Work. The lifting and rigging plan shall be review and approved by
Company Representative. Contractor shall provide the heavy equipment construction/installation work.

Contractor shall provide minimum 150 ton crane to offloading and install skid ultrasonic metering
package. Contractor shall provide lifting plan as part of the heavy lifting plan in advance. The arrival of
metering skid will be July – October 2017, and the crane cot in the periods is including in the Contractor
lump sum. Company will notify Contractor 2 months prior delivery of the skid ultrasonic metering
package. Delay of crane mobilization will be subject to KPI at Exhibit F.

2.4.1.7 Working at Height & Temporary Working Platform

Full body harness shall be provided for the workers carrying out the work in high elevation and follow
Company specification. Temporary working platform shall be erected using certified scaffolding
materials in accordance to Company Document No. ID-G-BU-00000-00000-00433 - Specification
Scaffolding Materials. Scaffolder and Scaffolder Inspector shall also have valid certificate as per
Company HSE standard and/or regulation.

2.4.1.8 Hot Work

All hot works to be performed inside Company facilities shall be approved by Company Authorizer as
per Company Permit to Work (“PTW”) System. Positive pressure habitat shall be used instead of
normal fire blanket if Site Risk Based Assessment (SRBA) result indicates the requirement.

2.4.1.9 Back Charge

In the event Contractor and/or Subcontractor fail to meet the Contract Schedule of the Contractor and/or
Subcontractor’s scope of work and scope of supply, Company shall have a right to source
materials/services from other parties. All incurred costs shall be deducted accordingly from the Contract
Value (considered as negative change order to Contractor).

2.4.1.10 Logistic

Contractor shall prepare required logistics to support all activities related to the Work including:
1 Laydown area/warehouse, completed with the security, security guard and fencing, to accommodate
all Contractor’s and Company’s supplied equipment and materials;
2 Temporary accommodation for the Contractor’s personnel. Contractor shall be responsible for
providing temporary accommodation for the remaining Contractor’s personnel;
3 Transport of Contractor’s personnel between temporary accommodation and the Grissik Station
which meets Company HSE standard and/or regulation;
4 Transport of Contractor’s personnel, equipment, and materials inside the Grissik Station;
5 Temporary office for Contractor’s personnel in the Grissik Station;
6 Provision and transport of fuel, food, water and other consumables to temporary accommodation
and the Grissik Station; and
7 Removal of garbage, trash and human waste from temporary accommodation and the Grissik
Station.

2.4.1.11 Transportation

Contractor shall provide all equipment and services required for the transportation of all personnel,
equipment and materials from/to/inside Grissik Station. Contractor shall map the route to the site, and
secure any permits required to haul an oversize load and the like to Grissik Station. Contractor shall
evaluate the entire access road network. Contractor shall provide to Company an adequate
transportation plan. Any improvement work or reinforcement required for the transportation of all
equipment and materials from/to/inside Grissik Station as well as for the movement of heavy vehicles
shall be responsibility of Contractor.

Contractor shall provide Company with procedures that cover lifting and transportation methods for all
major equipment. Contractor shall provide valid certification for all lifting equipment including all
auxiliaries. These procedures are subject to Company approval prior to use. Contractor shall manage all
licenses, permits, approvals or other authorizations, as may be necessary from the local, provincial,
regional, or national governments or other regulatory bodies to transport all materials and equipment’s
to Grissik Station.

Contractor shall handle all materials and equipment in a manner that will prevent damage. Lifting gears,
cranes, fork lifts, trucks, trailers, etc. shall be properly sized and in good condition, and operated by
experienced and qualified operators. Materials and equipment shall not be dragged, dropped, pushed or
rolled in a manner that could cause damage to the items. Contractor shall provide any support for
Company furnished material transportation of Ultrasonic Metering package, including heavy equipment,
personnel, and expertise from Company Mangsang Port to Grissik Plan. All transportation arrangements
shall comply with Company’s Journey Management Document No. ID-G-BU-00000-00000-00399.
Driver for light vehicle and heavy vehicle shall have valid certificate/training and obtain Company’s
driving licence.
Fabrication

Contractor shall provide the following, as a minimum:


1. A complete yard facility capable of undertaking all aspects of the fabrication phase of the Work,
including testing;
2. Fabrication workshop office, stores, warehouses, materials and equipment, yards, parking, and the
like;
3. Personnel and equipment for satisfactory operation of the workshop facilities and for the receipt,
handling, storage and preservation of each and every item of construction material and equipment;
4. All necessary scaffolding required to complete the Work. Erection of scaffoldings at the existing
facility will require Company approval;
5. All temporary lifting, slinging, and rigging equipment required including its operator. All such
equipment and operator shall be tested and certified by Certifying Authority in accordance with
Company’s requirements; and
6. Assurance that loose material is regularly dampened in order to prevent it from being blown into
stored materials or onto the Work, the fabrication site shall be maintained in a clean, dust free and
orderly condition.

2.4.1.12 Installation

Contractor shall complete all installation activities to meet requirements set out in the scope of work.
Contractor shall provide all equipment, materials, consumables, manpower, and services required for
the installation and tie-in of all equipment and piping including lifting equipment, supervision, labour,
purge gas facilities and the like. Contractor shall have qualified engineering personnel present during
installation to make sure all works are completed as per approved design specifications and drawings.

2.4.1.12.1 General Installation Requirements

Contractor shall provide the following items:


1 Maintain a continuous record of the sequence and the characteristics of each installation activity;
2 Confirm correct alignment of all piping and equipment;
3 During installation, ensure that the pipe curvature and stresses are within acceptable limits;
4 Ensure that the agreed monitoring and control measures are followed;
5 Ensure all activities are carried out in accordance with drawings, specifications and procedures;
6 Provide the qualified personnel, acceptable tools and equipment and as agreed with Company;
7 Follow the approved installation sequence and obtain written approval from Company prior to any
deviation;
8 Perform lifting, lowering into position, and bolt-up of each item of equipment, and each pipe spool
to its relevant foundation or support; and
9 Responsible for the supply of all special tools and equipment necessary for installation.

2.4.1.12.2 Piping Installation Works

Contractor, or its delegation, approved mechanical and piping subcontractor, shall provide all
procedures and documentations as needed for all mechanical and piping installation and tie-in works.
The mechanical and piping installation works shall include but not be limited to:
1. Perform dismantle and demolish of unnecessary existing piping safely as per approval Drawing;
2. Installation of all pipework connecting equipment, including pipework which ties in the existing
facilities to the new equipment;
3. Installation of interconnecting piping on the equipment skid (e.q Ultrasonic metering package and
nitrogen bottle package); and
4. Installation of manual valves and other pipework accessories (e.g. strainer, spring support, spectacle
blind, spade, etc.).
Related to the piping tie-in, Contractor shall refer to the latest version of Tie-in Schedule. Contractor
shall perform all required tie-ins with minimum disruption to plant operation. Contractor shall prepare
and provide tie-in plan or strategy to minimize the down time and have the plan in line with the
shutdown schedule from the Company. Since most of mechanical and piping installation will be
performed during normal/live plant, Contractor shall anticipate the use of proper fire blanket or even
positive pressure habitat as per Company HSE standard and/or regulation. Contractor shall perform Non
Destructive Test for new piping installation as per Company specifications.

Construction Method for Ultrasonic Metering Package are as follows:


1. Site preparation work for the compressor prior the erection and installation:
a. Perform check the skid foundation , make sure the level of the skid for the compressor has
already level, no crack on the grouting or any defect on the foundation which may cause crack
on the foundation as result to high compressor load;
b. Perform check on the gantry overhead crane function and capacity, check the position of the
gantry crane to the skid perspective and the erection/ drop point of the compressor to ensure no
obstacle, make sure if any lifting accessories for the crane provided by the compressor
compatible with the gantry crane;
c. Perform check on the laydown area to ensure true distance on the area and the surrounding
consistent with lifting study and rigging plan prior to erection. Check the ground bearing at the
erection location to ensure the crane path;
d. Perform check on the surrounding erection area to ensure no structural and equipment
obstructing the crane path and the lifted equipment in the collision line; and
e. Perform coordination walk around check join with Grissik Operation to ensure all the
paperwork and minimum requirement meets clients’ standard.

2. Preliminary check on the equipment prior to erection and installation:


a. Perform check on the compressor and accessories crates to the warehouse upon arrival using
given packing list/delivery number etc., Assist Supplier /perform material check on compressor
condition and the accessories using Supplier check list and then store the equipment in the
intended area as per specification; and
b. Perform check on the existing to ensure the existing facilities has been completely isolated and
fully protected against collision should there be any mishap during erection. Perform the check
on the existing piping and electrical/ Instrumentation wiring and all instrumentation
surrounding the compressor drop area to ensure all has been LOTO and secured.

3. Erection and Installation:


a. Contractor to ensure all the supporting accessories for the erection and installation are fully
functional and compatible with the new equipment;
b. Ensure that contractor has completely reviewed and fully understand the Installation Guideline
provided by Supplier;
c. Ensure that erection is done as per Supplier guideline;
d. Perform the levelling and install necessary shim pad if required for levelling;

e. Ensure all the necessary kit/hub and connection for the tandem compressor are already in place
for installation;
f. Ensure the installation of the coupling, spacers, accessories and the alignment is as per
installation guideline by Supplier; and
g. Ensure the connection re-assembly from the existing compressor to the new compressor unit is
as per Supplier guideline.

2.4.1.12.3 Instrumentation, Control and Automation Installation

Contractor, or its delegation, approved instrument and control specialist subcontractor, shall provide all
procedures and documentations as needed for all instrumentation and control installation and tie-in
works. The instrument and control installation works shall include but not be limited to:
1 Installation of Control Valves, Actuated On/Off Valve, Flow Element, Pressure Safety Valves;
2 Installation of all new instrumentations for Grissik-Pertagas Ultrasonic Metering and Pipeline Tie-
in Project including field instrumentations (transmitters, gauges, etc.), instrument cables, junction
box, cable tray and cable tray support, instrument support, and pneumatic instrument tubing. All
new transmitters shall be HART based;
3 Recalibration on existing Grissik Plant and Wells field instrument due to change on operating
conditions; and
4 Run and terminate instrument cables from field instrumentations through junction boxes to existing
PCS, SIS marshalling cabinet as well as perform the cross wiring from marshalling cabinet to I/O
Card.
5 Contractor shall use existing vendor PCS/SIS (Emerson / Delta V) to complete all the work related
to PCS system, SIS system, software installation and modification, DCS work, and other work to
integrated between DSC/SIS to the metering control system.
6 Contractor shall use the expert fibre optic termination, testing, splicing, integration, and data
transferring problem solving.
7 Contractor shall provide the expert of metering system package expert to integration between
metering control system, existing metering control (SSWJ) and DCS/SIS.
8 Testing, loop test, calibration, and other instrumentation work.
All instrument and control installation work shall comply with Company Electrical Equipment in
Hazardous Area (“EEHA”) procedures. Company has the right to audit all instrument and control
installation.

2.4.1.12.4 Electrical Installation Works

Contractor, or its delegation, approved electrical specialist subcontractor, shall provide all procedures
and documentations as needed for all electrical installation, tie-in, pre-commissioning and
commissioning/start-up works. The electrical installation works shall include but not be limited to:
1 Installation of supplementary normal and emergency lighting fixtures (if required);
2 Installation/termination of lighting cables (if required); and
3 Installation of grounding system.
4 Termination all the power system
5 Ligthening system installation
6 Tie-in work to the existing system
7 All testing and interconnection test required.
8 Other work related the electrical system.
All electrical installation work shall comply with Company EEHA (Electrical Equipment in Hazardous
Area) procedures. Company has the right to audit all electrical installation.

2.4.2 Fabrication, Construction and Installation Non Shutdown

The construction and installation of the Work that not specified in the shutdown period window will be
as part of Non Shutdown Work.

2.4.2.1 Pertagas Ultrasonic Metering

2.4.2.1.1 Civil and Structure Works

Civil and structure Works for Pertagas ultrasonic metering during non-shutdown window shall include
the following activities:
1 performance of the pre-cast and in-situ Work for the pipe support foundations;
2 transportation and installation of pre-casted pipe support foundation;
3 performance of pre-cast Work for the Pertagas ultrasonic metering shelter foundation;
4 transportation and installation of pre-casted of Pertagas ultrasonic metering shelter foundation;
5 pre-fabrication of pipe support structure;
6 Installation of pre-fabricated pipe support structure;

Pertagas ultrasonic metering package shelter in accordance with Company requirement under
document No. ID-G-BU-DD3-STS-GN-98-0002 - Coating Specification for Offshore and Onshore
Facilities;
11 performance of the soil excavation, backfilling and compacting for foundation installation and other
associated Works such as demolition, land clearing and grounding system installation;
12 demolish existing paving block access way;
13 re-assembly the existing SSWJ ultrasonic metering package shelter to its original;
14 provision and pre-fabrication of new fences to replace the demolished existing fences;
15 demolish the existing fences;
16 installation of new fences as mentioned in item 14 above;
17 pre-fabricate and install the instrument item stanchion, junction box and distribution panel supports;
18 performance of inspection and test in accordance with Company approved ITP and Inspection and
Test Report (“ITR”); and
19 re-instatement of damaged existing facility due to fabrication, construction and installation
activities.

2.4.2.1.2 Piping Works

Piping Works for Pertagas ultrasonic metering during non-shutdown window shall include the following
activities:
1 performance of the piping spools pre-fabrication Works at Contractor’s fabrication shop;
2 performance of the piping spools field-fabrication Works at Company site which shall be carry out
using the positive pressurized habitat in accordance with Exhibit M;
3 transportation and installation of pre-fabricated piping spools, valves and other items with flange
connection type in adherence with flange management system requirement;
4 installation of special item as part of piping spool;
5 performance of lifting, installation of Pertagas ultrasonic metering package and performance of
fitting, alignment and connection with the piping system;

8 re-instatement of damaged existing facility due to fabrication, construction and installation
activities.

2.4.2.1.3 Instrument, Control & Automation Works

Instrument, control & automation Works for ultrasonic metering Pertagas during non-shutdown window
shall include the following activities:
1 installation of field instruments at site along with each accessories and supports;
2 installation of field junction Boxes at site along with each accessories and supports;
3 installation of new Pertagas ultrasonic metering package panel in which consist of flow computer,
HMI station, PDC server, data logger PC, switches, firewall, fiber optic patch panel and each
accessories inside of metering SSWJ building;
4 modification of data connection to the existing analyzers i.e. Gas Chromatograph (“GC”), H2S
analyzer and H2O analyzer to new ultrasonic metering package panel. Contractor shall also perform
the following :
a. Perform the addition to the serial communication card for GC;
b. Perform the addition of analog signal splitter for H2S analyser and H2O analyzer;
5 install the instruments cable trays and its accessories;
6 install (include pulling and termination) the instrument cables and its accessories;
7 install (include pulling, splicing, tie-in and testing) the fiber optic cables data and its accessories
from metering panel to Central Control Room;
8 install the instrument tubing and its accessories;
9 install the tubing trays and its accessories;
10 install the new grounding system which shall be integrated with existing grounding system;
11 install the new bonding system for all field instrument devices and junction box;
12 install the pneumatic actuators of manual valves;
13 conduct the inspection and test for all instrumentation component;
14 conduct the inspection and test for the instrument in accordance with Company approved ITP and
ITR;
15 provide the cable entry/holes on gland plate for cable access to Power Control Room (“PCR”);

include software downloading and uploading;
17 perform data retrieval from new ultrasonic package panel to DCS;
18 perform the pre-commissioning of instrumentation devices which are connected to Flow Computer,
DCS, and SIS;
19 perform the gas metering system integration Test based on Company approved procedure and
acceptance criteria prior to witnessed by DITMET/DIMET dan MIGAS. In case of test result does
not meet the acceptance criteria, Contractor shall responsible for conducting the troubleshooting;
and
20 perform the gas metering system integration Test based on Company approved procedure and
acceptance criteria witnessed by DITMET/DIMET dan MIGAS. Support re-calibration activities
and prepare the calculation report as part of DITMET/DIMET and MIGAS certification. Contractor
shall provide all pressure and temperature standard calibration tools including the experience
technician

2.4.2.1.4 Electrical Works

Electrical Works for ultrasonic metering Pertagas during non-shutdown window shall include the
following activities:
1 install (including pulling and termination) the normal and emergency lighting power cables and
associated accessories;
2 install (including pulling and termination) the Pertagas ultrasonic metering package power cable
120 VAC and its accessories as following details :
a. from UPS to UCP located at SSWJ metering building; and
b. from UPS to new power distribution panel located at ultrasonic metering package shelter;
3 install (including pulling and termination) the Pertagas ultrasonic metering package power cable
220 VAC and its accessories from distribution panel to UCP at metering building;
4 install (including pulling and tie-in/termination) of Pertagas ultrasonic metering package power
cable 24 VDC and its associated accessories from new power distribution panel to Pertagas
ultrasonic metering package;
5 install (including pulling and termination) the receptacle power cable with the 208 VAC and its
associated accessories;
6 performance of inspection and test in accordance with Company approved ITP and ITR; and
7 install the new grounding system which shall be integrated with existing grounding system;

appropriate buildings;
11 perform the existing panel modification including the installation of new circuit breaker;
12 relocate the existing lighting pole, lighting junction box and its accessories;
13 Install the new power distribution panel, the accessories and its supports located at Company
Grissik site ;
14 Install new lighting at Pertagas ultrasonic metering package shelter;
15 uninstall Duri ultrasonic metering package shelter existing lighting and provide the lighting
temporary facility during performance of modification Work, re-instate the existing lighting to
original condition;
16 uninstall SSWJ ultrasonic metering package shelter existing lighting and provide the lighting
temporary facility during performance of modification Work, re-instate the existing lighting to
original condition;

1 perform the pre-fabrication and assembly Work for the extension of 30” sales gas header;
2 perform the pre-fabrication and assembly Work for the extension of 2” vent gas header line;
3 perform the assembly Work for instrument air supply header tie-in;
4 performance of the coating and painting Work for aforementioned pipe spools as in accordance with
Company document No.ID-G-BU-DD3-STS-GN-98-0002 - Coating Specification for Offshore and
Onshore Facilities;
5 performance of inspection and test in accordance with Company approved ITP and ITR; and
6 perform the identification, assessment and preparation for the replacement of orifice plate of 25-
BDV-0404.

2.4.3.1.3 Instrument, Control & Automation Works

b conduct the gradual leak test by using the sales gas up to operating pressure;

2.4.4.1.2 Instrument, Control & Automation Works

2 Closely monitor the construction activity to ensure the adherence with Company HSE requirement.

2.5 Mechanical Completion, Pre-Commissioning, Handover, Commissioning & Start up Assistance

2.5.1 Mechanical Completion

Contractor shall turn over the Facilities to Company for acceptance in accordance with Exhibit D –
Completion of the Work. Such acceptance shall be documented with certificates having Checklists and
Punch lists attached as appropriate. In order to achieve a proper and correctly documented Facilities
Acceptance, Contractor shall have the dedicated team for Facilities Acceptance and assign a Facilities
Acceptance Coordinator who shall be approved in advance by Company. The Facilities Acceptance
Coordinator shall possess a complete knowledge and understanding of all requirements pertinent to the
approved Facility Acceptance Procedures from Mechanical Completion through Complete Facility
Handover.

The Facilities Acceptance coordinator shall have responsibilities as follows:


1 implement, monitor, control, and record the passage of documents from Contractor to Company and
from Company to Contractor; and
2 Maintain a close interface with the Company Facilities Acceptance Coordinator and such that a high
level of confidence and understanding can be achieved between all concerned parties.

All surplus Company Furnished Material (including permanent and temporary material) and surplus
Contractor’s Supply (which Company has paid for Contractor) as may be available shall be handed over
and delivered to Company’s designated location or should be disposed of as directed by Company.

2.5.2 Pre-commissioning

Contractor shall conduct the pre-commissioning Work for all installed equipment, piping, instrument
and electrical items. This includes check on the integrity of all connections and gaskets, and all
functional testing. Pre-Commissioning activities shall include but not necessarily be limited to the
following:
1 Cleaning, final checking and boxing up of new equipment;
2 Flushing and blowing of all pipework;
3 Nitrogen leak test up to 100Psig;
4 Oxygen removal to the new system (and existing system which related to the new system;
5 Touch-up of all painting and protective coatings as required;
6 Megger testing of instrument and electrical cables;
7 Continuity, loop, function test for all instrumentations;
8 Testing and calibration of all field instrumentations;
9 Operational check of all control systems, as much as possible;
10 Functional testing of all new equipment, flow control valve, shutdown valve, actuated on/off valve
and manual valve;
11 Function test on the ultrasonic metering skid facilities with assistance from the Supplier;
12 provision of gross leak testing, flange management, instrumentation checking, and testing in overall
EPC scope and the ultrasonic metering package as part of the per-commissioning activities;
13 Provide all consumable, bulk, tools, nitrogen and all accessories as part of the pre-commissioning
all the system including ultrasonic metering system; and
14 Provision of temporary connection e.g. hose, tubing, pipe spools, nitrogen header completed with
pressure gauge, quick coupling and other associated components.

All material and equipment/tools, consumable and manpower for all pre-commissioning activity listed
above shall be under Contractor responsibility. All pre-commissioning activities shall be witnessed by
Company representative and properly documented for a basis to apply for Mechanical Completion.
Contractor shall be responsible to provide Supplier support of each material and equipment supplied to
assist mechanical completion, pre-commissioning, and commissioning activities, at no additional cost to
Company.

2.5.3 Handover

Contractor to perform the Handover of Contract object to Company after completing Mechanical
Completion and pre-commissioning refer to the Company detail requirement as stated under Exhibit D.

2.5.4 Commissioning and Start-up Support

Contractor shall provide some personnel (minimum one piping team and one instrument/electrical team)
to assist Company during commissioning/start-up. All Contractor personnel shall be under Company
supervision during commissioning/start-up period. Contractor shall provide nitrogen for leak test up to
target operating pressure for the whole system.

Contractor’s responsibilities shall only include but not be limited to:


1 Supply of commissioning spare parts;
2 Supply nitrogen;
3 Supply of all materials, equipment and consumables required in commissioning;
4 Supply of manpower and expertise required to assist in commissioning;
5 Coordination of Suppliers required to assist in commissioning;
6 Provide service for integration instrumentation and electrical system between the DCS and the
ultrasonic metering station;
7 Provide all the consumable, tools, nitrogen and all accessories as part of the commissioning overall
system including ultrasonic metering system and inlet header shutdown;
8 Provide all support for gas in during flange management in all area including for the skid metering
system;
9 Integration overall system, DCS system, SIS system, ultrasonic metering control system, and site
instrument and electrical system;
10 Conduct instrument calibration, electrical test, instrumentation test overall system including
ultrasonic metering package system require during commissioning/start up, performance testing,
and the certification activities; and
11 Support all the documentation for site acceptance test, certification, and performance test of the
ultrasonic metering package and the shutdown system.

2.6.1 Optional Work

The optional work including:


1 Provision of 50 ton crane during 30 calendar days period. The crane shall include hiring cost,
mobilization and demobilization, two (2) back-to-back certified operators, consumable (fuel, oil,
etc.), tw0 (2) back to back mechanics, tools and two (2) back to back riggers, certified lifting gears
(sling, webbing sling, shackle, hook, bar, etc.). The crane shall be as per Company requirement
under ID-G-BU-GG0-STR-GN-00-0005_R2 – Mobile Crane Specification under Exhibit G.
2 Provision of ultrasonic metering system expert at Site for the duration of thirty (30) calendar days.
The personal qualification shall be as follows :
c. The designated personnel shall be gas custody metering system (ultrasonic meter, flow
computer and all accessories) principal’s expert;
d. Familiar with Company’s gas custody metering system brand that used in this Contract;
e. Contractor shall provide mobilization and demobilization services, legal permit (visa, work
permit, local and government permit etc.), accommodation (housing and meals) and
transportation from Jakarta airport to Site and vice versa, transportation during provision of the
Work.
f. The personnel shall comply with Company’s HSE standard for working Company’s site
3 Provision of Nitrogen for the execution of shutdown and commissioning and start up Work with the
following details :
a. two (2) ea. isotanks (with the capacity of 12,000 barrels);
b. vaporizer, headers, 100 meters flexible hose and connection accessories; pressure indicator,
flow indicator and other instrument accessories;
c. the isotank drivers, vaporizer operators, helpers and mechanics shall comply with Company
HSE standard.
d. provision of fuel, consumable for the equipment and trucks;
e. accommodation (meals and housing) and transportation for all personnel as defined in item c
above.
4 Installation, termination, splicing, testing of fiber optic cables for data logger metering system from
South Sumatera West Java (“SSWJ”) metering room to battery limit of PT. Pertagas. Both of
facilities are located in Company’s Grissik plant with the length of about 1,000 meters. Company
will provide single mode 12 core armor fiber optic and its accessories.

Pipeline Tie-In Project

5 supply and install the utility installation interface with PT. Pertagas tie-in point which comprise the
following :
a. instrument air line (2 inch ANSI/ASME 150# pipes fittings, flanges, bulks and valves) with the
estimate length of 200 meter;
b. potable water line (2 inch ANSI/ASME 150# pipes fittings, flanges, bulks and valves) with the
estimate length of 200 meters;
c. 4 core 1/0 AWG power cable for lighting with the estimate length of 1000 meters;
Contractor shall perform optional works as requested and directed by Company. Company shall have no
obligation to compensate or otherwise provide relief to Contractor for any optional work or undertaken
in advance of proper written Company Approval.

2.6.2 Other Services

This other services is intended to cover additional work and services that is required to support the
original scope of work to fulfil requirement in execution of the Work. Contractor shall perform other
services as requested and directed by Company in accordance with Exhibit I of this Contract.

Company shall have no obligation to compensate or otherwise provide relief to Contractor for any other
services initiated or undertaken in advance of proper written Company approval. Company may request
Contractor, in writing, to arrange and/or procure any unidentified provision of goods and services. Such
items may include, but not limited to, any procurement and construction Works. Contractor shall use
Unit Rates provided under Exhibit E of this Contract.
