Dear Health District II Office,

We are Grade 12 students of St. Paul University-Manila and currently taking the last two subjects in our
senior year, namely (1) Inquiries, Investigations and Immersion [3I’s] and (2) Work Immersion [WI].

As part of our WI (following DepEd order #30 series 2107 Work Immersion Delivery Model A) and in
relation to the paper we will write for 3I’s, we are required to observe, identify, and describe the nature of
a company, its products and services, its target clientele. In Particular, we would like to observe and conduct
interviews among patients and staffs in the following areas: (1) OPD (2) OB WARD (3) PEDIATRIC
WARD. And most specially the company’s processes for safety, production, maintenance, quality control
and assurance, customer satisfaction, housekeeping, and hygiene for both personnel and clientele. All of
these should be accomplished within three to four days.

Hence, we would like to ask if we may be allowed to render four to eight hours a day in your company
on the dates which you will designate; preferably within these period: February 6- February 9. If you are
willing to assist us in our endeavor to prepare ourselves in our chosen fields, then kindly sign the conforme
