 PEME forms and required diagnostic tests must be taken to ship by crewmember.

Crewmembers arriving with

incomplete medical documents may be denied boarding and returned home or will be asked to repeat these tests
during the first voyage at their own expense.
 If eligible, returning crewmember may renew their medical on board if current medical valid on day of sign on.

Uric Acid , HDL, LDL, Total Cholesterol, Triglycerides)
Hemoglobin A1c is required if crewmember is Diabetic
Urinalysis, dipstick - micro if positive
Urine drug screen (Benzodiazepines, Amphetamines, Cocaine,
Marijuana, Opiates )
If HIV + then – viral load, CD4 count
Hepatitis B Surface Antigen (HBsAg)
Hepatitis A –Anti HAV IgM
Hepatitis C - Anti HCV
Syphilis Test – RPR or VDRL or TPHA
ECG only if high blood pressure or other cardiac issue
Proof of MMR Vaccination
Audiogram not required if whisper test is normal
