Addenda to the PaychekPLUS! Elite Visa Payroll Card (the “PaychekPLUS! Payroll Card or Payroll Card”)
Cardholder Account Agreement for applicable Payroll Card Savings Account owners.
These PaychekPLUS! Payroll Card Savings Account Terms and Conditions (the "Agreement") amend and
supplement the PaychekPLUS! Elite Visa Payroll Card Cardholder Account Agreement (the "Primary
Agreement"). If there is a conflict between this Agreement and the terms and conditions of the Primary
Agreement, this Agreement controls. As used in this Agreement, the words "you" and "your" mean each
person in whose name the PaychekPLUS! Payroll Card Savings Account (the "Payroll Card Savings
Account") is held or who exercises an ownership interest therein and any assignee or successor in interest
to the Payroll Card Savings Account. "We," "us," "our," and "PaychekPLUS! Elite Visa Payroll Card" refer to
the Payroll Card Program Service Agent as established and authorized by Darden Employees Federal
Credit Union (the "Financial Institution"), and, where appropriate, the Financial Institution. Your Payroll Card
Savings Account application will be accepted or declined by the Financial Institution. Receipt of this
Agreement and continued use of your Payroll Card Savings Account means that you agree to all of the
terms of this Agreement. We reserve the right to change any of the terms and conditions of this Agreement
and other disclosures or requirements by providing advance notice as may be required by law for any
change that is adverse to you. By maintaining your Payroll Card Savings Account after the effective date of
any change, you agree to be bound by any such changes. The PaychekPLUS! Elite Visa Payroll Card
Savings Account is available only to PaychekPLUS! Elite Visa Payroll Card cardholders.
This agreement is subject to applicable federal laws and the laws of the state of Florida (except to the extent
that this agreement can and does vary such rules or laws). The body of state and federal law that governs
our relationship with you, however, is too large and complex to be reproduced here. The purpose of this
document is to: (1) summarize some laws that apply to common transactions; (2) establish rules to cover
transactions or events which the law does not regulate; (3) establish rules for certain transactions or events
which the law regulates but permits variation by agreement; and (4) give you disclosures of some of our
policies to which you may be entitled or in which you may be interested.
To access your Payroll Card Savings Account funds (the "Funds"), you must transfer the Funds from your
Payroll Card Savings Account into your Debit Card Account by visiting www.paychekplus.com (the "Web
Site") or by calling 1-877-659-1881 ("Customer Service"). Transfers from your Debit Card Account shall be
the sole source of funding for your Payroll Card Savings Account. You cannot deposit funds directly to your
Payroll Card Savings Account. There are no maximum balance limits on your Payroll Card Savings Account.
All transfers to and from your Payroll Card Savings Account will be free, and your Payroll Card Savings
Account is not subject to an inactivity fee. Should fees ever be imposed on your Payroll Card Savings
Account, they will be deducted from your Payroll Card Savings Account.
As a holder of a Payroll Card Savings Account, and in compliance with applicable federal law, you may
make no more than six transfers from your Payroll Card Savings Account to your Debit Card Account per
quarter by means of a pre-authorized, automatic, telephonic or online banking order or instruction. No
transfers may be made from your Payroll Card Savings Account by check, draft debit card, point-of-sale or
similar device. A customer-service representative may perform a seventh transfer in emergency situations;
however, in this case, all Funds will be transferred to your Debit Card Account and your Payroll Card
Savings Account will be closed. Transfers of funds into your Payroll Card Savings Account are not limited,
except as set forth in this Agreement. Per Internal Revenue Service ("IRS") regulations, if your Payroll Card
Savings Account accrues more than $10 in interest for the calendar year; we will mail you an IRS Form 1099
in January of the following year.
Right of Offset
If you owe us money for any reason, and it becomes due, we reserve the right under the law (called Right of
Offset), and under this agreement to use money from your Payroll Card Savings account to pay the debt and
any cost or expenses incurred by us in enforcing our rights. Any such indebtedness owed to us by you, may
be charged to any account which is in your name or held jointly in your name and the name of another or
others. We may exercise this right to set-off at any time without prior notice to you or any owner of such
account. If we make a set-off against any of your Accounts, you agree to release and indemnify us from all
liability for our actions.
Instant Transfers
In addition to the automatic recurring fund transfer option, you can make one-time instant transfers to your
Payroll Card Savings Account by visiting the Web Site, or via the automated voice response unit or by
calling Customer Service and speaking to a Customer Service Rep. These transfers are real-time and post
to your Accounts within 30 minutes. Any transactions that cannot be completed using a voice response unit
will be completed by a live agent, which may incur a fee.
Fees
There are no fees associated with your Payroll Card Savings Account; however, in the event that the Funds
from your Payroll Card Savings Account are transferred to your Debit Card Account, any fees associated
with your Debit Card Account would apply.
Withdrawals
In order to withdraw Funds from your Payroll Card Savings Account you must first transfer the Funds to your
Debit Card Account, from which you may access your Debit Card Account funds pursuant to the terms of the
Primary Agreement. If your Debit Card Account has a negative balance when this transfer occurs, any fees
due on your Debit Card Account will be collected at that time.
Account Closure
You may close your Payroll Card Savings Account from this website or calling Customer Service. When you
close your Payroll Card Savings Account, all Funds will be automatically transferred to your Debit Card
Account, at which time any outstanding fees on your Debit Card Account will be collected. Any accrued
interest will be forfeited. To claim your Funds, you must withdraw the Funds from your Debit Card Account.
We will not send checks for remaining Payroll Card Savings Account balances. We may suspend or close
your Account if we suspect illicit, fraudulent or suspicious activity on any of your Accounts with us. If we
close your Account(s), we will return to you via check all available funds, net of fees or uncollected
transactions. If your Savings Account is closed, you may not open another Savings Account until the first
day of the following month. If the balance in the Savings Account is $0.00 for 90 consecutive days, we may
close your Savings Account, regardless of the status of the Card account. We may also close your Savings
Account if you withdraw your consent to receive electronic periodic statements. If we close your Savings
Account you may lose any accrued but unpaid interest.
Your consent applies to the Electronic Fund Transfer Disclosure and the Truth in Savings Disclosure. You
may withdraw your consent to receive the disclosures electronically, In order to withdraw your consent, you
must write us at Cardholder Services, P.O. Box 551617, Jacksonville, FL 32255.
In order to receive the disclosures electronically the following minimum computer hardware and software
requirements must be met: IBM compatible or Macintosh personal computer system with Internet
connectivity; available hard disk drive space of at least 50 kb; a computer pointing device, such as a mouse;
128-bit SSL (Secure Socket Layer)-compatible browser, such as Internet Explorer 5.0, or higher; computer
monitor capable of displaying Internet Web pages and graphics; graphical user interface operating system,
such as Windows 2000 or later, or OS9.
By opening a Payroll Card Savings Account with PaychekPLUS! Elite Visa Payroll Card via this website, you
certify that you agree to the Payroll Card Savings Account terms and conditions, that you consent to receive
disclosures, agreements and instructions electronically, that you have not withdrawn said consent and that
you agree to the terms and conditions of the Primary Agreement.
Documentation
You may access your Payroll Card Savings Account history and/or periodic statement 24/7 by visiting the
Web Site or calling Customer Service. You will have access to a continuously updated Account Statement
(the "Statement") on the Web Site. This Statement is available to you regardless of how often you use your
Payroll Card Savings Account. Should you wish, we will make a paper statement available to you upon
request for a fee of $2.50 per request in which the fee will be deducted from your Payroll Card Savings
Account.
Variable Rate
The interest rate for your Payroll Card Savings Account is 2.00% with an annual percentage yield of 2.02%.
Your interest rate and annual percentage yield are subject to change.
Determination of Rate
We may change your annual percentage yield and corresponding interest rate at our discretion.
Fees
There are no additional fees associated with your Payroll Card Savings Account; however, in the event that
the Funds from your Payroll Card Savings Account are transferred to your Debit Card Account, any fees
associated with your Debit Card Account would apply.
Transaction Limitations
As an Accountholder, and in compliance with applicable federal law, you may make no more than six

Transfers of funds into your Payroll Card Savings Account are not limited, except as set forth in the Terms
and Conditions.
Our Privacy Policy and Practices recognize the trust you have placed in us. We are committed to preserving
that trust and we will work hard to safeguard the confidentiality of your personal information. Of course, we
reserve the right to modify this policy to reflect any changes or enhancements that we make to those
protections. If we do make modifications, we will let you know promptly.
Information We Collect
We may collect information about you from the following categories and sources:
Information you provide to us on applications or other forms (examples include name, address, social
security number, assets and income).
Information about your transactions with us, our affiliates or others (examples include your account
balances, payment history, debit card usage).
Information collected from your browser when you visit our websites and your browser interacts with them.
Information we receive from consumer reporting agencies (examples include your credit worthiness and
credit history).
As required by the USA PATRIOT Act, we also collect information and take actions necessary to verify your
identification.
The Fair Credit Reporting Act allows you the opportunity to limit the sharing of “nonexperience” information
within our family of affiliated companies. Such information includes information that is used, expected to be
used or collected for the purpose of establishing your eligibility for consumer credit, insurance and related
products and services, and is not related solely to your transactions or other communications with us. If you
would prefer that we not share nonexperience information with our family of affiliated companies, you may
direct us not to share this information by calling us toll-free at 1-877-659-1881 or write us at Cardholder
Services, P.O. Box 551617, Jacksonville, FL 32255.
If you opt out of our sharing of “nonexperience” information with our family of affiliated companies, we may
still share information relating solely to your transactions or other communications with us with our affiliates
to allow such affiliates to market additional services to you. If you would prefer that we not share such
experience information with our family of affiliated companies for such purpose, you may direct us not to
share this information by calling us toll-free at 1-877-659-1881 or write us at Cardholder Services, P.O. Box
551617, Jacksonville, FL 32255.
Regardless of these opt out elections; we may still share information in other ways that are permitted by law,
for example, as needed to service your relationship with us.
To the extent that we share your personal information as described above with service providers or joint
marketers, such disclosures will be subject to an appropriate, contractual confidentiality pledge, and solely
for the purpose and only to the extent necessary to carry out the contractual purposes, or as otherwise
permitted or required by law.
Dormant and Abandoned Accounts: The Card will be considered dormant when there has been no
activity on the card during the past twelve months. Unless prohibited by law, the Card may be charged an
inactivity fee as described in the Schedule of Fees. If a deposit or withdrawal has not been made on the
Card within the period specified by state law, the Card will be presumed to be abandoned. The Issuing
Bank will notify you of the unclaimed property, as required by law, at your last known address. The funds
on the card will no longer be accessible to you, unless you contact us by calling 1-866-906-9328. Funds
in abandonment will be reported and remitted in accordance with state laws. Once the funds have been
turned over to the state, the Issuing Bank has no further liability to you for such funds and if you choose to
reclaim such funds, you must apply to the appropriate state agency.
