AUTHORIZATION TO CHECK EMPLOYEE IDENTITY, CRIMINAL

& EMPLOYMENT REFERENCE

As a part of due diligence, SOURCEEDGE requires a background investigation and a

check of references to be conducted. It is expressly understood that any information
given is to be used for the purpose of determining my acceptability for employment with
SOURCEEDGE.

I understand that SOURCEEDGE has authorized Rezorce~Check to conduct such

investigations on behalf of SOURCEEDGE. I understand that Rezorce~Check is a
professional agency conducting such investigation in India.

By my signature below, I voluntarily authorize SOURCEEDGE to share information of

my background, education, employment & earnings history to Rezorce~Check. I
understand that Rezorce~Checkand its officers will check my references by
contacting any person they deem as appropriate references. These include my present
and previous employers, learning institutions [including school & college], law
enforcement agencies and Federal, State & Local Courts.

I also hereby release SOURCEEDGE and Rezorce Managed Solutions Pvt. Ltd., from all
liabilities for damages or claims – which may arise or result from any
referenceinformation gathered pursuant to this authorization.

I have completed the attached “Employment Information” form on my own. I swear that
all information is true to my knowledge and belief
