Concern 1: The Fong family feels overwhelmed and uneducated with what down syndrome includes and

what can be done to help their daughter. This is the Fong families’ first child and first grandchild and the
family doesn’t have any background experiences with disabilities.

Current Resource/Information: Mr. Fong has knowledge on child development from his background of
being a pediatrician.

Outcome: The Fong family will have the knowledge they desire regarding learning more about down
syndrome.

Suggested Action: Mr. and Mrs. Fong will read through the National Down Syndrome Society webpage.
More specifically the Down Syndrome section and the New & Expectant Parents sections.

seeks to educate communities and families about Down Syndrome and to provide support and
advocacy.

· Policy Information – An NDSS Information & Referral Specialist is available to respond to emails and
phone calls between the hours of 9am and 5pm ET, Monday - Friday. The helpline is supported by a
translation service that can be accessed in over 150 languages.These services are toll-free.

Rationale: This website gives the Fong family more information about down syndrome.

Concern 2: Mr. Fong was given a promotion so the Fong family will be moving to Milwaukee, Wisconsin,
which is two hours away from their current residence. Mrs. Fong will be the one responsible for looking
for a new home.

Hollman about homes in the area.

Outcome: Mrs. Fong will be able to find the new home for her family.

Rationale: This information is for her real estate agent, this will help make house buying go smoother for
Mrs. Fong.

Concern 1: Mrs. Fong is feeling alone and needs social and emotional support. Mrs. Fong is looking for a
support group that she can participate online because she feels uncomfortable talking in groups about
their difficulties.
Current Resource/Information: Mrs. Fong has visited the site for Family Voices but doesn’t know much
about the support group.

Outcome: Mrs. Fong will be able to find a support group online so that she can feel comfortable
receiving support without discussing her concerns in person. They also want to find a support group for
social reasons and activity opportunities to get to know other families by having fun rather than
discussing the challenges they face.

Rationale: The support groups are for Mulan as well as the Fong Family. The local support group called
GiGi’s playhouse is for the families to get involved with other families in activities. Parent to Parent is
more focused on allowing the parents to get one on one support from other parents that have “been
there”. Family of voices is family centered and the support lasts until Mulan would be in her adulthood.
The support groups provide friends and families that Mr. and Mrs. Fong can meet with and receive
advice and emotional support. Mulan will also form friendships with other children in the groups. The
support groups will give the family and Mulan a social aspect to facing their difficulties.

Concern 2 : Mrs. Fong’s mother lives with them and is currently taking a course in respite care to get
certified.

Current Resource/Information: Grandma Fong has raised a child before, and so she has a knowledge on
how to care for children.

Outcome: Grandma Fong would be able to get certified in respite care and funding would be provided
for the care, in order to give Mrs. Fong a break.

Suggested Action:Grandma Fong will apply for Respite Care and go through the course needed to
become certified.
