Dear Sir/Madam,

Re: Ms.Nina Sharman, DOB:09/02/1951

I am writing this letter to refer this patient, who requires urgent assessment and management
on the swallowing ability and nutritional status, (place a comma here) as she is potential
for aspiration.

Ms. Sharman, who is single, is a new resident of our institution under the protection of
Australian Guardianship and Administration Council. She had an episode of choking, (no need
to place a comma here) yesterday, (comma is not needed here) on a piece of food, which is
assumed to have not been chewed. After the removal of solid food, she was relieved from all
the choking signs; hospitalization was not required as all parameters were back to normal.

Ms. Sharman seems to have increased appetite as she was found to have eaten food from other
residents' room, after consumption of her whole meals. Moreover, her weight has been
increased, over the last 5 months, by 10 Kg with a BMI of 30. She can only follow simple
instructions as she has dementia; she sometimes refuses to wear her upper and lower dentures
because of confusion and disorientation.

It would be greatly appreciated if you could visit her and check her swallowing reflux as well as
nutritional pattern to avoid any further complications.
