Phytonadione

Usual Diluents

D5W, NS (diluents should be preservative-free)

Note: Infuse slowly; rate of infusion should not exceed 1 mg/minute (3 mg/m2/minute
in children and infants). Alternatively, dilute dose in a minimum of 50 mL of
compatible solution and administer using an infusion pump over at least 20 minutes
(Ageno, 2012). The injectable route should be used only if the oral route is not
feasible or there is a greater urgency to reverse anticoagulation.

WARNING — INTRAVENOUS AND INTRAMUSCULAR USE


Severe reactions, including fatalities, have occurred during and immediately after
INTRAVENOUS injection of phytonadione, even when precautions have been taken
to dilute the phytonadione and to avoid rapid infusion. Severe reactions, including
fatalities, have also been reported following INTRAMUSCULAR administration.
Typically these severe reactions have resembled hypersensitivity or anaphylaxis,
including shock and cardiac and/or respiratory arrest. Some patients have exhibited
these severe reactions on receiving phytonadione for the first time. Therefore the
INTRAVENOUS and INTRAMUSCULAR routes should be restricted to those
situations where the subcutaneous route is not feasible and the serious risk involved is
considered justified.

Stability / Miscellaneous

Injection: Store at 15°C to 30°C (59°F to 86°F).

EXP: Administer shortly after preparation.
