This application form has been designed to help us understand and evaluate your proposal to
become and remain a valuable Channel Partner for UML.

Guidelines

1. Kindly go through the complete form thoroughly before filling.

2. Application form should be filled in English language only.

3. It may be noted that the “APPLICANT" here refers to the main person who would be managing
daily operations of the proposed dealership.

4. All the financial information mentioned should be in Rupees in Lacs.

5. The application form should be duly filled in all respects. Please do not leave any blanks or leave
answers incomplete.

1) The applicant certifies that he/she has made full disclosure of all the information required in this application and all the
information given by him/her is true in all respects. He/ She further acknowledges that any mis interpretation,
whether intentional or unintentional, in the information submitted in connection with the application shall be the
ground upon which UML may either:(a)Decide to no longer consider his application OR(b) Immediately terminate any
dealership agreement executed by UML with him/her or the dealer company designated by him/her.

2) This applicant acknowledges that this is only application for dealership and by submitting this application; he/she will not
place any obligations on UML to appoint the applicant as an authorized dealer of UML.

3) The applicant acknowledges that any obligations undertaken or expense incurred by him in submitting this application
shall be incurred entirely on the applicant's own behalf.

4) The applicant hereby authorizes UML to make any inquiries it may consider necessary to verify this information and will not
consider such inquiry to constitute an invasion of his privacy.

5) The applicant acknowledges that copies of all documents, photographs etc .furnished by him / hereto UML in the process of
applying for dealership would hence forth become a property of UML and may be retained by them for their records.

6) The applicant hereby authorizes the receipt, provision, verification, and exchange of relevant credit related information
from I with any source including banks, credit rating agencies, and credit bureaus in respect of his personal account as well as
business for the purpose of considering his request.

IMPORTANT NOTICE
The submission of your Dealer Application Form is only the first step in the process to qualify for UML
Dealership. Until you are officially informed in writing and are presented with a Dealership Letter of
Intent, you are not appointed as a UML Dealer. Only after completion of Terms & Conditions in the LOI
& after due diligence by UML Team, we shall enter into a Dealer Agreement for official commencement
of authorized UML Dealership Operations.
