    Parents’ Waiver of Permission

    This is to certify that I am allowing my <son, daughter>, <Full name of the student> to go on an
    immersion as mandated by the Department of Education for a minimum of eighty (80) hours
    starting < start date> until <assumed end date> at <name of company>. I understand that this
    immersion program is a partial fulfilment to the course Accountancy, Business and Management.

    It is tacit that he/she abides by the rules and regulations that may be imposed by the company thus
    in turn imploring his/her safety and welfare as the priority.

    I fully agree to waive any responsibility on the part of Asian College Inc., the principal and
    teachers-in-charge, <name of the company>, and/or other representatives in case of any untoward
    and uncontrollable incident that may happen to my <son, daughter> in the duration of the
    immersion.
