position of Attorney III, and currently assigned to this department’s Legal Service.

I would like to humbly seek permission, and respectfully request from your
good office for the AUTHORITY TO ENGAGE IN THE LIMITED PRACTICE OF LAW, to
wit: as Notary Public, for and in the City of Manila, and as Private Counsel.

I undertake to abide by the limitations and to follow the rules set forth in
Section 12, Rule XVIII of the Revised Civil Service Rules, and other relevant and
pertinent laws, rules and regulations in relation to the aforementioned authority.
