School Flex Program Student Application
WHAT IS IT?
The School Flex Instruction Program is a focused program that will allow students to take a lower
course load with proof and verification of employment. This program is focused on students in 11th and
12th grade.
GUIDELINES
Eligible students who participate in the School Flex Instruction Program must comply with the
following guidelines:
* Hoosier Academies High School will follow work permit guidelines with the expectation that students
will understand that academics must come first.
THE PROGRAM
1. Student must fill out “School Flex Instruction Program Application” and provide proof of
employment prior to starting the semester.
2. Principal and grade level school counselor will approve submission, course student according to
needs, and notify student of approval and coursing.
3. Student must maintain good standing with school and maintain employment throughout the
time they are in the School Flex Program.
4. Counselor will verify employment every 9 weeks

FLEX PROGRAM STUDENT APPLICATION
Student must meet the following requirements to be participate in the School Flex Program (initial by
each requirement):
(If student is 18 years old or older, then only the student needs to sign the form.)
Thank you for your interest in the FLEX program. All acknowledgement forms must be completed
and be submitted with the student application form. Complete forms must be submitted in order
for the administration to review the application.
If approved, a notice will be sent to student applicant by email welcoming them to the FLEX program.
