submitted it, your Official Document Request to hand to a school official, and instructions
on how to have your ACT/SAT scores sent to the University of Utah. Keep this file for
your records.
STEP 2: Take your Official Document Request to your school official. Take the following
Official Document Request to the school official who sends your official transcripts
(registrar or school counselor) and ask that the appropriate materials be submitted to the
University of Utah as soon as possible.
STEP 3: Pay your application fee. You can log back in to your application to pay online, or send
a check or money order to:

collegeboard.org (our school code is 4853). You can submit your ACT scores at
act.org (our school code is 4274).

When I was four, I started dancing. I went to a neighborhood dance studio until I was 15 where I did clogging, jazz, hip hop,
ballroom. As I got older, I decided to focus on ballroom and joined a more competitive studio. At this studio, my team competed
in different competitions and won on a national level in swing category. Since I was five years old, I have taken piano lessons
from my great-grandmother. I have come to appreciate the strengths of the engineering program at the University of Utah. I
love the vibe at the university and would love to be part of it as a student. I can contribute to the diverse campus community by
being a woman who is interested in engineering and ballroom dance.
