Provided face to face medication training and support services for Tamara K Chandler today Phoenix

Place. The writer is a nurse for Midtown and rendered serves for Tamara. Tamara brought medication
from the outpatient pharmacy and her medication box was reconciled for 7 days. Self-administration
medication records were prepared along with a controlled substance log. Tamara was educated on
medication times, process and procedures of the Phoenix Place programs.

Place 1.5 tablets under the tongue daily

DULoxetine (CYMBALTA) 30 mg capsule


Take 1 capsule (30 mg total) by mouth daily

Take 1 capsule (60 mg total) by mouth daily

atorvaSTATin (LIPITOR) 40 mg tablet


Take 1 tablet (40 mg total) by mouth daily

cloNIDine (CATAPRES) 0.2 mg tablet


Take 1 tablet (0.2 mg total) by mouth at bedtime

albuterol HFA (PROVENTIL HFA;VENTOLIN HFA) 90 mcg/actuation inhaler


ONE OR TWO PUFFS QID PRN SOB

Benefit/Focus of the service:


Medication education, risk/safety to prevent hospitalization and or incarceration. The writer discussing
side effects of not taking her medication correctly and not sharing her medication with others in the
community. Integration into the community.

Goal from Treatment Plan:


Tamara will develop and utilize the coping strategies on a daily basis to manage symptoms related to
depression and anxiety as evidenced by reported decreased sadness/worry and increased feelings of
hope.

Progress Towards Goal:


Tamara was able to fill her medication box with minimal assistance. With ongoing medication
adherence Tamara will display adaptive coping strategies with help from her treatment teams.

Plan/Next Steps:
The writer will continue to render services to Tamara as needed or requested by her team.
