Work, Other Animals and Breeding Stocks Property Card

INSTRUCTIONS

A. This Property Card shall be used by the Property Office to

record details on the acquisition, birth of offspring and
disposition of work, other animals and breeding stocks.
B. This form shall be accomplished as follows:
1. LGU – name of the province/city/municipality.
2. Fund – Fund to which acquisition and /or income
from disposition was charged/recorded.
3. Kind of Animal – classification of animal, i.e. work
animal, other animal or breeding stocks.
4. Acquisition – details of acquisition of animals, i. e:
Date : Date of acquisition
Property Number : Property Number assigned
by the GSO/Property
Officer to the animal
Description : Description of the animal
such as color, breed,
brand, Natural Marks,
blemishes, etc.
Quantity : Number of animals acquired
5. Birth of Offspring – details of birth of offspring. i.e:
Date : Date of Birth
Number of Offspring : No. of live births
Property Number : Property Number
Assigned by the GSO
/Property Officer to
the animal
Description : Description of the animal
such as color, breed,
brand, Natural Marks,
blemishes, etc
6. Disposition – details of disposition of animals and
offspring, i.e.
Date : Date of disposition
Property Number : Property Number
Nature of Disposition : Mode of disposition such
as sale, death, transfer,
donation
Number Disposed Of : Quantity of animal
disposed of
