Directions

1. Print out on card stock or paper. This page makes two of the small Hart Charts.

2. If you have very young patient’s or others who are easily distracted by their surroundings, cut out the
square at the top of the chart. Instruct them to look through this when they read the big chart. The
paper margins help hide some of their surroundings and the physical act of looking through a specific
location tends to “center” them on the large chart.

3. For those patient’s who are NOT easily distracted, the “window” is not needed. Simply cut or fold
the paper till only the chart is visible, leaving some blank space at the bottom for them to hold the chart
in their fingers without blocking the print.

4. Note: This chart was made with a 12 pt. Lucida Console font. If you wish to make other size print
charts for any reason, just select the 2 charts with your computer mouse, paste them to a new document,
and change the font size to something larger or smaller.
