The horizontal growth of the midface is determined by the expansion of the anterior cranial
fossa, which enlarges anteriorly to an extent that matches the sagittal development of its
equivalent, i.e. maxilla (Fig. 44). The resorption and deposition processes on the endocranial
and ectocranial surfaces lead to displacement and remodeling of the underlying structures
(nasal bone, ethmoid bone). Thus, the horizontal development of the midface is balanced
inferiorly with the anteroposterior elongation of the maxilla and superiorly with the
horizontal extension of the anterior cranial fossa.
During vertical displacement of the midface, the maxillary arch is displaced in a downward
direction due to resorption on its nasal surface and simultaneous, deposition on its palatal
surface (Fig. 45) Thus, space is created for the sinuses to expand. Their inner surfaces are
largely resorptive. Unlike other primates, the anterior part of the human maxilla is resorptive,
and the premaxilla grows straight downward and does not move forward (Fig. 45).
Apart from these remodeling processes, the vertical and development of the maxilla is also
associated with its primary displacement (Fig. 46), the latter triggering an equivalent amount
of secondary sutural bone deposition (Fig. 46). The downward movement of the maxilla is
usually not parallel, but differs anteroposteriorly. This applies to both the remodeling process
and the displacement. The result is a rotational movement of the maxilla, which can be
compensated or increased by the sum of variations in both growth mechanisms (Fig. 46).
Due to active apposition and resorption processes in the bony alveolar sockets (Fig. 47),
vertical drift of the upper teeth sets in simultaneously to maxillary remodeling. Further
lowering of the upper dentition results from displacement of the maxilla (Fig.47) This type of
downward movement of the dental arch can, above all, be influenced by therapeutic means.
The structural counterpart for the downward growth of the nasomaxillary complex is the
vertical enlargement of the ramus and middle cranial base, as these growth processes displace
the mandible inferiorly (Fig. 42). Once the upper teeth have moved into place, an upward
drift of the lower teeth and their alveolar processes commences, thus establishing the final
occlusion (Fig. 4S). As the vertical movement of the teeth is physiologically greater in the
maxilla than in the mandible, the orthodontic therapeutic possibilities are also greater in the
maxillary arch.
Simultaneous to the drift of the lower teeth, remodeling takes place around the chin, the
mandibular corpus, the lower incisors, and their alveolar processes. The incisors rotate
lingually and move posteriorly to establish the proper overbite.
Resorption on the outer surface of the anterior alveolar region and bone deposition on the
anterior and inferior external contours of the symphysis cause the chin to become more
prominent (Fig. 48)

The bones of the midface are enlarged and displaced simultaneously. Deposition on the oral
cortical plate and resorption on tha nasal cortical surface cause the palate and premaxilla to

This process takes place parallel to the remodeling process of the hard palate and premaxilla

The lower teeth drilt, together with their alveolar bone, in an upward direction, thus
establishing the occlusion. The lower incisors and their alveolar process move lingually due
to remodeling growth. At the same time, new bone is deposited around the chin.
Growth Equivalents
The Hunter-Enlow growth equivalents concept is an important principle covering the
development of the facial skeleton. The individual components of the skull develop in
different directions, they must interreact directly, in order to compensate for the various
growth activities. This is achieved by growth equivalents which act in opposing directions.
These growth equivalents coordinate the different movements of the cranial base, the
nasomaxillary complex, and the mandible, which are due to development, and thus determine
the adaptive changes in relation to the individual parts of the skull. For example, elongation
of the anterior cranial base is related with enlargement of the nasomaxillary complex.
Disturbances during realization of this growth pattern cause craniofacial anomalies. The
distrubance can be related to disproportions of the equivalents in the vertical or horizontal
plane.

Pertumbuhan Midface
Pertumbuhan horizontal midface ditentukan oleh ekspansi fossa kranial anterior, yang
membesar ke anterior ke tingkat yang sesuai dengan perkembangan sagital pada
ekuivalennya, yaitu maxilla (Gambar 44). Proses resorpsi dan deposisi pada permukaan
endocranial dan ectocranial menyebabkan perpindahan dan remodelling struktur yang
mendasari (tulang hidung, tulang ethmoid). Dengan demikian, perkembangan horizontal
midface seimbang secara inferior dengan pemanjangan anteroposterior dari maksila dan
superior dengan ekstensi horizontal dari fossa kranial anterior.
Selama perpindahan vertikal midface, lengkungan rahang atas dipindahkan ke arah ke bawah
karena resorpsi pada permukaan hidung dan bekerja secara bersamaan, deposisi pada
permukaan palatumnya (Gambar. 45) Dengan demikian, ruang tsb diciptakan untuk sinus
berfungsi untuk memperluas. Permukaan bagian dalam mereka bersifat resorptif. Tidak
seperti primata lain, bagian anterior rahang atas manusia adalah resorptif, dan premaksila
tumbuh lurus ke bawah dan tidak bergerak maju (Gambar. 45).
Terlepas dari proses remodeling ini, vertikal dan perkembangan rahang atas juga terkait
dengan perpindahan primernya (Gambar 46), yang terakhir memicu jumlah deposisi tulang
sutural sekunder yang setara (Gambar 46). Gerakan ke bawah dari rahang atas biasanya tidak
paralel, tetapi berbeda anteroposterior. Ini berlaku untuk proses remodeling dan pemindahan.
Hasilnya adalah gerakan rotasi rahang atas, yang dapat dikompensasikan atau ditingkatkan
dengan jumlah variasi dalam kedua mekanisme pertumbuhan (Gambar. 46).
Karena proses aposisi dan resorpsi aktif pada soket alveolar tulang (Gambar 47), pergeseran
vertikal gigi atas secara simultan menjadi remodeling maksilaris. Penurunan lebih lanjut dari
hasil dentisi atas dari perpindahan rahang atas (Gbr.47) Jenis gerakan ke bawah dari lengkung
gigi ini dapat, di atas semuanya, dipengaruhi oleh sarana terapeutik. Mitra struktural untuk
pertumbuhan bawah kompleks nasomaxillary adalah pembesaran vertikal ramus dan basis
kranial tengah, karena proses pertumbuhan ini menggantikan mandibula secara inferior
(Gambar 42). Setelah gigi atas telah pindah ke tempatnya, pergeseran ke atas gigi bawah dan
proses alveolar mereka telah dimulai, oklusi akhir dibentuk(Gambar 4). Karena gerakan
vertikal gigi secara fisiologis lebih besar pada rahang atas daripada pada mandibula,
kemungkinan terapeutik ortodontik juga lebih besar pada lengkung rahang atas.
Bersamaan dengan pergeseran gigi bawah, remodeling terjadi di sekitar dagu, korpus
mandibula, gigi seri bawah, dan proses alveolar. Gigi seri memutar lingual dan bergerak ke
posterior untuk membentuk overbite yang tepat.
Resorpsi pada permukaan luar daerah alveolar anterior dan deposisi tulang pada kontur
eksternal anterior dan inferior simfisis menyebabkan dagu menjadi lebih menonjol (Gambar.

Tulang-tulang midface diperbesar dan berpindah secara bersamaan. Deposisi pada piring
kortikal oral dan resorpsi pada permukaan kortikal hidung menyebabkan langit-langit dan

Ekuivalen Pertumbuhan
Konsep pertumbuhan setara Hunter-Enlow adalah prinsip penting yang mencakup
pengembangan kerangka wajah. Komponen individu tengkorak berkembang dalam arah yang
berbeda, mereka harus melakukan interreaksi secara langsung, untuk mengkompensasi
berbagai kegiatan pertumbuhan. Hal ini dicapai oleh ekuivalen pertumbuhan yang bertindak
berlawanan arah.

horizontal.
