3. Your mom and stepdad’s certificate of marriage, if they are married and if
your biological parents were never married

4. Affidavit of consent of your biological father (I can prepare this for you, so
as with the other Affidavit of consent no. 5 and no. 6)

5. Affidavit of consent of your stepdad and mom’s children, if any, who are
10 years old or above

6. Your Affidavit of consent since you are already above 10 years old

7. Home Study Report (this will be accomplished by a social worker)

8. Your stepdad’s:

 Medical Certificate
 Barangay Clearance
 Police Clearance
 Municipal Trial Court of Malasiqui Clearance
 NBI Clearance
 Certificate of Employment or other documents showing his work
 Income tax return / Payroll
 Bank account/certificate, if any
 Titles of properties in his name, if any
