Buddy Evaluation

Objective:
 To receive feedback from the New Hire how his / her buddy helped the New
Hire to settle into the departments and begin to function by themselves.
 To also identify the best buddy for the quarter by getting feedback from the
Executive Committee as well as Department Heads.

Scoring:
Give a rating of 5 for very good, 4 good, 3 average, 2 fair, 1 poor

Questions:
 Did the buddy introduce himself / herself to you?
 Did he or she make you feel welcomed?
 When you were taken to your department, did your buddy take time to
introduce you to everyone in your department?
 Did he or she give you a tour of your department?
 Did he or she take time to explain the New Hire Orientation check list?
 Did he or she take time to explain the Technical Skills Training & Tracking
record?
 Did he or she schedule a 15 minute training for you?
 Did he or she follow the Technical Skills Training schedule properly?
 Did he or she give you constructive feedback regularly to help you improve
your performance?
 Did he or she ask you for feedback or suggestions to improve service
operations?
