 Details of Medical Examination to be conducted

Note - For the epilepsy test, the doctor will ask the driver about medical and personal history and whether he uses
any medicines, drugs or alcohol. On the basis of the information shared by the driver, the doctor may be able to
make a diagnosis of epilepsy. Clinical test like electroencephalogram (EEG) or magnetic resonance imaging
(MRI) could not made part of the examination.

Commercials – Rs. 750/- + GST @18% (Per Driver)

Terms and Conditions

1. GST would be charged at 18%.

2. Payment Terms: Payment would be released within 7 days of the program by Cheque/DD payable at
par to: Safety Circle Pvt. Ltd.
3. Client has to arrange for room for conducting the examination in plant.
