Medical Leave Letter

Warmest greetings from me, Kuhan s/o M.Salvan@Murugan your 4th year student writing this
letter to you, Dr. Jidon to formally inform the reason for my absence to Ethics class on
9/11/2017 (Thursday). This is because I had a bad diarrhea for the past 2 days from 8/11/2017
till 9/11/2017. Then I went to Warga Clinic UMS for medical checkup and the medical officer
has confirmed that it is because of some food poisoning stuff.

The medical officer who carried out the medical procedure for me has entitled me with 1 day of
medical leave as prior to my health concern and as I was unable to attend the class because of
the non-stop diarrhea.

Your kind consideration is highly appreciated Dr. in accepting my medical certificate which I
attach with this letter for your reference Dr.
