BREAST

No rashes, infection and unusual pigmentation. Not enlarged symmetric breasts, no discharge with lightly
pigmented areolae and nipples. No supernumerary nipples noted.

CHEST & LUNGS

No chest wall deformity, discoloration, or masses. Nasal flaring, grunting and irregular
breathing pattern (periodic respiration) noted on respiration. Compliant and symmetric chest
wall expansion with supraclavicular, subcostal, substernal, and intercostal retractions. No tactile
fremitus on breathing. Stridor heard on all lung fields both anterior and posterior wall. No
wheezes, rales and crackles noted.

HEART
No acrocyanosis. Adynamic precordium without thrills. Normal rate & rhythm. Murmur noted at
the left upper sternal border 2nd ICS, intensity Grade 2 that does not increase in inspiration and
expiration. PMI not appreciated.

ABDOMEN
Protuberant; umbilicus brown, midline, inverted, without discharge. No redness or swelling
around the umbilicus. No prominent vessels, striae and visible pulsations. Normoactive bowel
sounds. Liver palpable at 2.5cm below right costal margin; spleen and kidneys not palpable.

PERIPHERAL VASCULAR
Extremities are warm on all extremities. Capillary refill time <1s.
