DITCHING OF PIPER AZTEC, RPC-5818

On February 4, 2018, Local time 1330, Piper Aztec PA23-250 with registration number of RPC-
5818 boarded with two crews, Pilot in command Harihalan Muniandy and co-pilot Max Harvey,
carrying live fishes cargo ditched by the shoreline of long beach, San Vicente Palawan.

Details of flight,

Prior to engine startup, complete 360 check and flight briefing established by both the pilots.
Approximately time 1305, both the engine started and engine run-up had been conducted with no
discrepancies were found on both the engines. Then, continued taxiing to line up for runway 04
straight out departure. Pilot in command executed a normal take off after completing take
procedures and checklist. Establishing positive rate of climb, landing gears were retracted
followed by a left climbing turn towards the heading of 025 due to terrains ahead. Approaching
120 feet, an abnormal sound was heard in the cockpit by both the pilots from theleft engine and
noticed power loss on the left engine with heavy vibration on the yoke. With the visual reference
and engine instruments indication, both the pilots identified left engine failure. A quick engine
failure procedures had been carried out by maintaining directional control of the aircraft and
throttle, propeller and mixture levers were fully forwarded and confirmed left engine failure.
Without further delay a right turn towards the live engine was made while trying to establish best
rate of single engine climb speed of 89 knots but unfortunately aircraft started losing altitude and
airspeed indicator indicated 70knots. Terrain ahead and right prohibited turn towards the live
right engine; unable to extend landing gear as hydraulic failure due to inoperative left engine and
lack of height and time prevented a manual landing gear extension. A decision was made to
make a 5 degree left turn and ditch close to shoreline. Left turn completed, confirmed aiming
point in relation to waves. At around 40 to 50 feet, the copilot unlocked and unlatched main door
to facilitate immediate evacuation. Both the pilots evacuated safely after the impact with water
by the shoreline. Tip tank debris was used as floatation aid and awaited rescue.
