1. I am the duly representative of Starboard Manpower Services

Inc., (SMSI), the complainant in the above-entitled case.

2. I caused the preparation of the foregoing Complaint.

3. I have read the foregoing Complaint, the allegations therein are

true and correct of my personal knowledge and/or based on
authentic documents.

4. I have not commenced any other complaint, action or proceeding

involving the same issue/s in any other Court, Tribunal or
