Greetings!

The Samahan ng mga Detinado ng Tarlac Provincial Jail (SDTPJ) is requesting

for the approval of the following activity for this month’s Family Day Celebration on
January 13, 2018 at 7:00 o’clock in the evening at the Bulwagan Stage. We will be
staging a Bingo Bonanza to generate initial funds for the SDTPJ and for the benefits of
all inmates. Such funds will proceed to projects and activities as planned by the
organization

SUBJECT : Request for Medical Officers for Medical Mission

DATE : November 24, 2016

1. Reference: Weekly medical check-up of inmates by Dr Raymund Talon of

Talon General Hospital on November 14 and 21, 2016.

2. Some inmates are suffering from illnesses such as diabetes, hypertension,

bleeding disorder, cough and colds, and uterine problems.

3. In view of the foregoing, may we request for medical doctors particularly OB-
Gynecologist and internal medicine doctors preferably on Mondays, Wednesdays, and
Fridays as scheduled non-visiting days at Tarlac Provincial Jail.
