AFFIDAVIT

I, Robiul Islam, son of Md. Nurul Islam of Village: Naduria, Post Office: Koshba Majail,
Thana: Pangsha, District: Rajbari, at present residing at House: 20/A, Road: 10, Sector:
7, Uttara, Dhaka-1230, aged above 21 years, by profession student, by faith Muslim, by
Nationality Bangladeshi by birth do hereby solemnly affirm and declare as follows:-

1. That my name mistakenly appeared in both of my S.S.C. and H.S.C. certificates

as “Robiul Kabir”.

2. That my name is Robiul Islam and as such I have made correction to my name
from “Robiul Kabir” to “Robiul Islam” before the Notary Public and from now my
name is “Robiul Islam”.

3. That the statement made above are time to the best of my knowledge and belief
and in witness whereof I sign this affidavit before the Notary Public on 1 st day of
January, 2011, Dhaka, Bangladesh.
