Statements & Search Criteria

Mission
To integrate interdisciplinary aging research with clinical capabilities to develop innovative programming that
can be applied and sustained by Riverside Health System, the community and other providers of aging-related
services to improve care and better meet the needs of a growing older adult population.

Background
We are a 501(c)(3) nonprofit organization created in 2002. Our focus is the provision of unique specialty
services for older adults and their caregivers. Through our academic partners we conduct health services
research to improve the skills of health practitioners and family caregivers who serve older adults.

Impact
To lead the development of model communities that will promote the well-being and independence of people as
they age through translational research and widespread application.

2016 Top accomplishments:


1.Expansion of caregiver support program and corresponding recognition at the state level;
2.Evaluation of telehealth programs to support nursing home residents and vulnerable older adults recently
discharged from the hospital
3.Effective geriatric interdisciplinary team to meet the needs of patients being served in our Geriatric
Assessment Clinic.

2017 Goals:
1.To incorporate care management into the Geriatric Assessment Clinic;
2.To expand offerings of our caregiver support programs to new communities in Virginia and to serve our health
system colleagues who are engaged in elder care; and
3.To secure funding to integrate dementia clinical guidelines in primary care sites
Needs
Top 5 Pressing Needs:
1.Test and integrate evidence-based models of care, specifically related to chronic conditions;
2.Secure state and federal funds ($600,000) to implement new programs and support salaries to deliver
programs;
3.Support community's awareness and proactive approach to the large demographic of older adults in our area;
4.Facilitate more education for professionals to better address the needs of persons with dementia and their
caregivers;
5.Develop partnerships with key academic and medical institutions including UVA and VCU.
CEO/Executive Director Statement
The Center, through its health services and applied research and provision of unique and special services for
older adults and their caregivers, works to create a model community for older adults and their caregivers.

• The Center was an independent organization before 2012. In 2012, it became a subordinate entity of
Riverside Healthcare Association and is included in their tax exemption. They file a group Form 990 with other
entities included in the group exemption.

solely of the Center.
