AFFIDAVIT

That We, of legal age, Filipinos and after being sworn to in accordance with law, do hereby depose and
say:

1. That I, SIA, FERDINAND J., the authorized representative of DOUBLE DRAGON PROPERTIES
CORP., have completed/renovated SKYSUITES CORPORATE CENTER, (building/structure) located
at LOTS 1&2, EPIFANIO DELOS SANTOS AVENUE COR. QUEZON AVENUE BGY. WEST TRIANGLE
QUEZON CITY.

2. That I, Engr. Ujinell J. Dimaano, a registered civil/structural engineer who was hired by the above
named owner and/or developer; to supervise the construction of the subject building/structure,
including all of its non-structural members and ancillaries.

3. That, we are applying for the Occupancy Permit of the above mentioned building/structure at
the Quezon City Department of the Building Official.

4. That, submission of the Material Test Result is a requirement of the Department in the
Occupancy Permit application.

5. That, the said Material Test Result could not be provided because the results got lost and cannot
be found despite diligent efforts to locate them.

6. That, despite of the lacking documents, we hereby attest that the materials used during
construction (Concrete/ Reinforcing Bars/ Structural Steel) conform to the approved structural
specifications of the project.

7. That, this affidavit is being executed to attest the truth of the above mentioned facts and for
consideration and approval of my Occupancy Permit application.

That We, of legal ages, Filipinos and after being sworn to in accordance with law, hereby depose and say:

That I, FERDINAND J. SIA, the authorized representative of the Double Dragon Properties Corp.,
the completed/ renovated SKYSUITES CORPORATE CENTER, (building/structure) located at LOTS 1&2,
EPIFANIO DELOS SANTOS AVENUE,BRGY. WEST TRIANGLE Quezon City.

That We, ENGR. ROLAND B. SANCHEZ & ENGR. UJINELL J. DIMAANO, registered civil/structural
engineers who were hired by the above named owner and/or developer, to conduct a comprehensive
structural analysis and design, evaluation, testing and supervision of the subject building/structure,
including all of its non-structural members and ancillaries and stability in order to ensure its safety and
fitness for human habitation and/or related purposes;

That, after the actual construction and implementation of its structural design, testing and
supervision, we have found the subject building/structure, including all of its non- structural members
and ancillaries to be sound and stable, and fit for human habitation and/or related purposes. Further, it
is compliant with the provisions and requirements mandated by the National Building Code of the
Philippines (NBCP) and its Implementing Rules and Regulation (IRR), otherwise known as “Presidential
Decree No. 1096”. Furthermore, it is compliant to the provisions and requirements of the latest edition
of the National Structural Code of the Philippines (NSCP), as a referral code of the (NBCP);

That we, hereby certify to the truthfulness and veracity of the above stated paragraphs, to jointly
assume any civil and/or criminal liability, in case of failures and/or damages incurred to the
aforementioned building/structure including all of its non-structural members and ancillaries to be sound
and stable, and fit for human habitation and/or related purposes, within fifteen (15) years from the date
of issuance of the Certificate of Occupancy/Use;
