    TRAINING FOR WORK SCHOLARSHIP PROGRAM


    SCHOLAR’S ATTESTATION

    I,___________________________________________, _______________, with postal address


    at _________________________________________________________________________do
    hereby attest:

    1. Undergo pre-qualifying process in accordance with industry pre-hiring


    requirements.
    2. Participate actively in the Training Induction of Scholars before the conduct of
    the training program.
    3. Attend the training program religiously and comply with the attendance
    requirements of the training program.
    4. Conduct myself in such a manner as not to bring disgrace or dishonor to TESDA
    and TVET provider I am enrolled in.
    5. Undergo the mandatory assessment prior to graduation (if there is available
    assessment instrument).
    6. Attend graduation ceremonies and other Scholar TVI activities.
    7. Participate actively in the career guidance and referral activities of the TVET
    provider.
    8. Not to change training program and Training Provider. Otherwise, I shall be
    disqualified from availing the scholarship.

    I hereby further attest that I have not been a recipient of any TESDA scholarship program from
    2006 to 2012.
