LIBRARY CARD APPLICATION

(Only for Incoming First Year and Re-entry)
1. For freshmen and re-entry:
 Must be officially enrolled
 1 pc.1x1 ID picture
 Duly filled up student record form to be secured at Table 2 In-charge of the
Library Card Application
 Photoshoot session for Attendance Management System (AMS) at Table 1
 User Registration in Library System

NOTE:
 Releasing of Library will be notified through SMS.
 Only those with library card are allowed to borrow/photocopy library books and will be allowed to transact
business with the librarian.
