9. Medical Certificate (CSC Form 211) with attached results of the Blood test,
Urinalysis, Chest X-ray and Drug test;
10. Photocopy of Certificate of Clearnce issued by the Office of the Court
Administrator, Supreme Court, and Notice of Acceptance of Resignation
issued by the Office of the Court Administrator;
11. Civil Service Commission Clearance;
12. Sandiganbayan Clearance.
