3. That I am acquainted with Sravan Muppavarapu. I was his supervisor.
4. During the period March 2012 until March 2013, I was employed with SA Anika
Technical and Consultancy Sdn.bhd
5. That I have direct personal knowledge of the event and circumstances concerning
the employment of Sravan Muppavarapu.
6. That from March 2012 until March 2013 Mr. Sravan Muppavarapu worked as a
Process Development expert, to my knowledge no less than 40 Hours per week.
7. I certify that as a Process Development Expert, Mr. Sravan Muppavarapu to my
knowledge performed the following duties:

Unit testing, support users during UAT and administration of data systems etc. In fact, he
was a key player in most of our projects. Develop innovative solutions to meet the needs of the

performance was satisfactory and work attitude was excellent.
