CONSENT, WAIVER & RELEASE

KNOW ALL MEN BY THESE PRESENTS:

That I, RYNNA RYNN V. CATRO, a Filipino, 22 years of age, a resident and post
office address at CURVA, SAN VICENTE, LILOAN CEBU, Philippines, and a student of
the University of San Jose-Recoletos, Cebu City, being a student of BACHELOR OF
SCIENCE IN CIVIL ENGINEERING in connection with the CE OJT – CE On the Job
Training, Seminars, and Field Trips, hereby manifest my consent to being such a
participant, and by these presents hereby waive, discharge and release the
UNIVERSITY OF SAN JOSE-RECOLETOS, its officers, members and employees from
any and all liabilities, cause or causes of action, sum or sums of money and damages
for any accident that may befall me during the entire period of training and that I sign
this CONSENT, WAIVER, & RELEASE freely and voluntarily and consciously with full
knowledge of my rights under the law.
