I, DR. ERNESTO FLORENCIO, of legal age, married, a government physician, and a


resident of Antipolo City, Rizal, after having been sworn to in accordance with the law do
hereby depose and state:

That Atty. Lady Stephen R. Barricuatro, a resident of Makati, Metro Manila City, is the
counsel who conducted and supervised my examination as a witness at her office at Rm. 4
2/F San Larazo Suites, Makati City, Metro Manila:

That I am answering the questions herein fully conscious that I do so under oath and
that I may be criminally liable for flase tstimony or perjury;

The following are the Questions propounded by Atty. Lady Stephen R. Barricuatro and
my answers in English language:

Q1: Please state your name and other personal circumstances for the record.

A1: I am Dr., of legal age, married, and a medical examiner for the Province of Rizal, and a
resident of Antipolo City, Rizal.

Q2: How long have you been practicing your profession?

A2: For almost twenty-five (25) years already.

Q3: Where were you presently employed?

A3: Antipolo Medical Center, Rizal City.


Q4: What position do you currently hold?

A4: Medical Specialist 4.

Q5: Why are you giving this testimony?

A5: I am here to testify on my examination of Luz Diaz who was confined last October 27-29,
2010.

Q6: Can you please tell this court your diagnosis and and the cause thereof.

A6: Luz Diaz suffered essential hypertension, partially controlled. This is a stress-induced.
Patients who usually suffer this kind of illness undergo severe stress, due to persistent
lack of sleep usually brought by their misery and depression.

Q7: Is it safe to assume that what the plaintiff suffered is due to the death of her beloved
dog?

A7: Yes.
