MEMBERSHIP AGREEMENT

THIS MEMBERSHIP AGREEMENT (the “Agreement”) is effective as of this _____

day of _____________, 20___ (the “Effective Date”) by and between (NAME OF
GROUP) and ____________________(“Member”).

Member agrees to pay the (annual/membership) fees established for its

membership;

Member agrees to abide by all policies and procedures adopted by (name of

group/membership/organization);

(name of group) shall have the right to suspend participation, or cancel the
membership, of Member if it fails to pay its (annual/membership) fees on
time. Thus, if a Member’s payment of its (annual/membership) fees is not
fully paid within a period of three (3) months from date of membership, its
membership shall be cancelled.
