SUBJECT : Processing of application for promotion for the CY 2012 first batch of
proposal for promotion.

Please be informed that processing of application for promotion for the CY 2012 first batch
shall soon commence. Applicants are enjoined to ensure that their respective applications
must be accompanied with the following minimum requirements:

1. Application Letter
2. Duly accomplished Personal Data Sheet
3. Individual Performance Commitment and Review Rating for CY 2017
4. Justification for promotion attested by applicant’s immediate supervisor
5. Work Assignment History duly singed by the applicant and certified by the head of
office
6. Authenticated Training certificates from the date of last promotion

On this regard, the Office shall still accept applications for promotion for this batch not later
than the last working hour of JULY 25, 2012. Applications received later than the cut-off
period shall be considered on the next batch of promotion.

Please be guided accordingly.
