ACKNOWLEDGMENT OF PATERNITY

I, NEIL MATURAN HALAWIG, 27 years old, single,

Filipino citizen and a resident of Purok-13B, Brgy. San Vicente,
Butuan City, under oath depose and say:

(1) That my common-law partner, Rhea Marie Pajo, gave

birth to our SON, Miguelito Pajo, in Butuan City on September 1,
2007, during which, we were not yet married;

(2) That I was not around to acknowledge paternity and sign

his birth records, as I was then working somewhere else;

(3) That until now my son, MIGUELITO PAJO, remain on

record as my illegitimate and unacknowledged child to legally
use my surname since me and his mother, Rhea, is not yet married;

(4) That I execute this affidavit to attest the foregoing fact

to ACKNOWLEDGE PATERNITY of my child, Miguelito Pajo,
and for such other legal purpose.

IN TRUTH WHEREOF, I hereby affix my signature this 24th

day of August 2016 in the City of Butuan, Philippines.
