AFFIDAVIT OF UNDERTAKING

I, GERLIE LACIA DELOS REYES, of legal age, Filipino, married and a resident of
Sasa, Davao City, Philippines, after being duly sworn to in accordance with the law hereby
depose and say:

That I was one of the Petitioners to take the Bar Examination last 2015 but I was not able
to take the said examination because of health reasons;

That I am executing this affidavit to request the Supreme Court or the government agency
concerned that I be given a discount from the payment of fees for taking the 2018 Bar
Examination and for whatever legal purposes it may serve best.
