AFFIDAVIT OF DISCREPANCY

I, BELLA P. RODRIGO, of legal age Filipino, widow with postal address at

San Zone 3, Sto. Tomas, Magarao, Camarines Sur, after having been duly sworn
to, freely and voluntarily depose and state:

1. That I am the mother of the late Josie Rodrigo Orcales who died
last June 1, 2014;

2. That there is a discrepancy in the death certificate of my

daughter Josie Rodrigo Orcales ;

3. That my name appearing in the death certificate is Bella P. Rodrigo which

is my maiden name instead of Bella R. Orcales;

4. That this discrepancy in my daughter’s death certificate is

inadvertently committed by the clerk who prepared the death
certificate;

5. That the person of BELLA P. RODRIGO who was born at

Magarao, Camarines Sur as appearing in the death certificate
of my daughter is one and the same person of BELLA R.
ORCALES, referring to myself;

6. That I execute this affidavit in order to attest to the facts for

all legal intents and purposes.

I, NOLASCO LIMCY ERTIZ, of legal age Filipino Citizen, married with

postal address at Himaao, Pili, Camarines Sur, after having been duly sworn to,
freely and voluntarily depose and state:

1. That I am the youngest son of SPOUSES FLORENCIO ERTIZ and

MARGARITA LIMCY;

2. That there is a discrepancy in my mother’s surname;

3. That my mother’s surname appearing in their marriage certificate

recorded as MARGARITA LIMCE which is her surname instead of
MARGARITA LIMCY;

4. That this discrepancy in my mother’s surname is inadvertently

committed by the clerk who prepared the marriage certificate of my parents;

5. That the person of MARGARITA LIMCE as appearing in the

marriage certificate is one and the same person of MARGARITA LIMCY;

6. That I execute this affidavit in order to attest to the facts for all
legal intents and purposes.
